肝细胞肝癌精准肝段切除术的疗效观察

Clinical observation of precise liver resection in patients with hepatocellular carcinoma

目的探讨肝细胞肝癌精准段切除术的临床疗效。方法选取2015年9月~2017年7月在我院诊断治疗的64例肝细胞癌患者,按不同的手术方式将其分为研究组、常规组,每组32例。研究组患者实施精准肝段切除术,常规组实施传统肝切除术,并将两组手术相关指标、术后肝功能指标及术后并发症情况进行比较。结果两组患者的术中输血量比较,差异无统计学意义(P>0.05);研究组患者术中出血量、手术时间、术后住院时间均显著低于常规组,差异有统计学意义(P<0.05);研究组患者术后并发症发生率显著低于常规组,差异有统计学意义(P<0.05);研究组术后ALB、ALT、TBil等肝功能指标均明显优于常规组,差异有统计学意义(P<0.05)。结论精准肝段切除术对肝细胞癌患者治疗效果显著,可有效减少术中出血量,缩短患者术后住院时间,减少术后并发症发生,改善患者肝功能,具备临床意义与实施价值。

Objective To study the clinical effect of precise liver resection in patients with hepatocellular carcinoma.Methods A total of 64 patients with hepatocellular carcinoma diagnosed and treated in our hospital from September 2015 to July 2017 were divided into study group and conventional group according to different surgical methods,32 cases in each group.Patients in the study group underwent precise segmental hepatectomy,while patients in the conventional group underwent traditional hepatectomy.The operation-related indicators,postoperative liver function indicators and postoperative complications were compared between the two groups.Results In blood transfusion volume between the two groups,the difference was no significant difference(P>0.05).The bleeding volume,operation time and postoperative hospital stay in the study group were significantly lower than those in the conventional group,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the study group was significantly lower than that in the conventional group,The difference was statistically significant(P<0.05).After operation,ALT,TBil,ALB and other liver function indexes in the study group were significantly better than those in the conventional group,the difference was statistically significant(P<0.05).Conclusion The precise liver resection has an obvious effect in patients with hepatocellular carcinoma,can reduce the intraoperative blood loss,shorten the hospitalization time,decrease the incidence rate of postoperative complications and improve the hepatic function.
