目前,社区卫生服务中心(站)主要任务,是承担辖区12大项基本公共卫生服务工作,而其中的慢性病管理,更成为了社区全科医生工作的重点和难点。如何解决这个难题,做好这项工作,成为了所有社区工作者探索的方向。家庭医生签约服务模式,为全科医生更好的管理慢性病提供了方向和目标。

Currently,the major task of community health service center(station)is shouldering 12 basic public health service jobs in the area under its jurisdiction,and the chronic management has become the difficult point and key point of community general practitioners,how to solve the difficult problem and do a good job has become the direction of all community workers,and the family doctors signing service model provides the direction and aim for the general practitioners to better manage the chronic diseases.
