营养不良、先天性心脏病与儿童重症肺炎的关联性分析

Association analysis of malnutrition,congenital heart disease and severe pneumonia in children

目的探究营养不良、先天性心脏病与重症肺炎患儿的关联性。方法选取2013年10月-2017年4月我院重症肺炎患儿115例作为研究组,并选取同期收治轻症肺炎患儿762例作为对照组。收集两组临床资料,如性别、年龄、是否为早产儿、人工喂养,是否伴有贫血、佝偻病、先天性免疫缺陷、反复呼吸道感染、先天性气道畸形及先天性心脏病,是否存在营养不良等,对儿童重症肺炎影响因素如营养不良、先天性心脏病与重症肺炎的关联性进行Logistic回归分析。结果年龄≤3个月、营养不良、贫血、先天性心脏病、先天性免疫缺陷及反复呼吸道感染均为儿童重症肺炎相关影响因素(P<0.05);年龄≤3个月、营养不良、贫血、先天性心脏病、先天性免疫缺陷及反复呼吸道感染均为儿童重症肺炎发病危险因素(P<0.05);营养不良、先天性心脏病均与儿童重症肺炎呈正相关(P<0.05)。结论年龄≤3个月、营养不良、贫血、先天性心脏病、先天性免疫缺陷及反复呼吸道感染均为儿童重症肺炎发病危险因素,且营养不良、先天性心脏病均与儿童重症肺炎呈正相关。

Objective To investigate the association of malnutrition,congenital heart disease and severe pneumonia in children.Methods 115 cases of severe pneumonia from October 2013 to April 2017 in our hospital were selected as the study group,while 762 cases with mild pneumonia were selected as the control group at the same time.The clinical data of the two groups were collected,such as sex,age,premature,artificial feeding,whether accompanied with anemia,rickets,congenital immunodeficiency,recurrent respiratory tract infection,congenital airway malformation and congenital heart disease,and malnutrition,etc.The influencing factors of severe pneumonia in children were analyzed.Then the association of malnutrition,congenital heart disease and severe pneumonia was investi-gated.Results Age≤3 months,malnutrition,anemia,congenital heart disease,congenital immune deficiency and recurrent respiratory tract infection were important related factors of severe pneumonia in children(P<0.05).Logistic regression analysis showed that age≤3 months,malnutrition,anemia,congenital heart disease,congenital immune deficiency and recurrent respiratory tract infection were the risk factors of severe pneumonia in children(P<0.05).Spearman test showed that malnutrition,congenital heart disease were positively correlated with severe pneumonia in children(P<0.05).Conclusion Age≤3 months,malnutrition,anemia,congenital heart disease,congenital immunodeficiency and recurrent respiratory tract infections are the risk factors of severe pneumonia in children,and malnutrition,congenital heart disease and children with severe pneumonia are positively correlated.
