经腹超声联合经阴道超声应用于孕中晚期前置胎盘诊断中的效果观察

Effect of Transabdominal Ultrasound Combined with Transvagial Ultrasound in the Diagnosis of Placenta Previa in Middle and Late Pregnancy

目的为了提高临床孕中晚期前置胎盘诊断效果,探讨经腹超声联合经阴道超声检查的价值和意义。方法选取经手术证实前置胎盘的患者30例作为研究对象,均分别接受经腹超声和联合阴道超声,以产后手术病理结果为诊断标准,比较两种检查诊断结果差异。结果术后病理证实30例为前置胎盘,其中单纯经腹超声确诊26例,诊断符合率86.67%;联合超声检查确诊29例,诊断符合率为96.67%,且两组比较差异具有统计学意义(P<0.05)。联合检查诊断符合率更占优势。与单独腹超声相比,联合超声检查在诊断特异度、灵敏度、阳性预测值及阴性预测值等方面更具优势,且比较差异有统计学意义(P<0.05)。结论孕中晚期疑似前置胎盘患者接受腹超声联合经阴道超声检查,有助于提高检出率,减少漏诊和误诊率,值得临床应用。

Objective To investigate the value and significance of the examination by using transabdominal ultrasound combined with transvagial ultrasound in order to improve the diagnostic effect of placenta previa in the middle and later stage of pregnancy.Methods 30 patients with placenta previa of pregnancy confirmed by surger were selected as the research subjects.All the patients were examined by transabdominal ultrasound and transvagiall ultrasound.The results of combined examination and single examination were compared with the pathological results of postpartum surgery as the diagnostic criteria,and the diagnosis results were compared.Results The research results showed that the postoperative pathological findings confirmed 30 cases of placenta previa.A-mong them,26 cases and 29 cases were diagnosed by simple transabdominal ultrasound and combined ultrasonography,and the diagnostic coincidence rates were 86.67%and 96.67%,respectively.The diagnostic rate of combined examination was more favorable as compared to single examination,and the difference between the two groups was statistically significant(P<0.05).Compared with transabdominal ultrasonography,the combined ultrasonography had more advantages in diagnosis specificity,sensitivity,positive predictive value and negative predictive value,and the difference was statistically significant(P<0.05).Conclusion Patients with suspected placenta previa at the middle and late stages of pregnancy need to receive transabdominal ultrasound combined with transperineal ultrasonography,which is helpful to improve the clinical detection rate,reduce misdiagnosis and misdiagnosis rate.This method is worthy of clinical application.
