空气压力波辅助血栓通、抗血小板治疗对脑卒中后下肢偏瘫患者偏瘫症状及下肢血流的影响 被引量:14

摘要 目的探讨空气压力波辅助血栓通、抗血小板治疗对脑卒中后下肢偏瘫患者偏瘫症状及下肢血流的影响。方法将2013年12月至2017年3月我院收治的88例脑卒中后下肢偏瘫患者随机分为对照组与观察组各44例,对照组采用血栓通、抗血小板治疗,观察组加用空气压力波辅助治疗,比较两组治疗效果及治疗前后下肢偏瘫症状的变化,测定两组神经功能及下肢功能的改善情况,监测患者下肢静脉血流速度、平均返流时间及腿围平均周径的变化,统计两组下肢静脉血栓形成发生率。结果治疗后,观察组肢体麻木、脉络淤血、偏瘫等症状评分低于对照组,治疗总有效率高于对照组(P<0.05);治疗后,两组美国国立卫生研究院脑卒中量表(NIHSS)评分降低,下肢运动功能评定量表(FMA)评分、Holden步行功能分级表(FAC)、10 min快速步行试验(10 m MWS)最快步行速度上升(P<0.05),观察组FMA评分、FAC、10 m MWS均高于对照组(P<0.05);治疗后,观察组血流速度高于对照组,反流时间短于对照组,腿围周径、下肢静脉血栓形成发生率低于对照组(P<0.05)。结论在脑卒中后下肢偏瘫患者的临床治疗中,采用空气压力波仪辅助血栓通、抗血小板治疗,可改善患者偏瘫症状,提高治疗效果,优化患者下肢功能,改善下肢血流,降低血栓形成发生风险。 To investigate the effect of air pressure wave combined with Xueshuantong and antiplatelet therapy on hemiplegia symptoms and lower limb blood flow in patients with lower limb hemiplegia after stroke.A total of 88 patients with lower limb hemiplegia after stroke treated in our hospital from December 2013 to March 2017 were randomly divided into control or observation group,44 in each group.The control group was treated with Xueshuantong and antiplatelet therapy while the observation group was additionally treated with air pressure wave.The curative effect and changes of hemiplegia symptoms before and after treatment were compared between the two groups.The improvement of neurological function and lower limb function in the two groups was determined.Changes of lower limb venous blood flow velocity,average backflow time and average thigh girth were monitored,and the incidence of lower limb deep venous thrombosis was statistically analyzed.After treatment,the scores of limb numbness,choroid congestion and hemiplegia in the observation group were lower than those in the control group(P<0.05).The total effective rate in the observation group was higher than that of the control group(P<0.05).After treatment,scores of the National Institutes of Health Stroke Scale(NIHSS)were decreased,while scores of lower limb motor function assessment scale(FMA),Holden walking function classification scale(FAC)and the fastest walking speed of 10 min fast walking test(10 m MWS)were increased in the both groups(P<0.05).However,FMA score,FAC and 10 m MWS in the observation group were higher than those in the control group(P<0.05).After treatment,the blood flow velocity was higher,the backflow time was shorter,and the thigh girth was smaller in the observation group when compared to the control group(P<0.05).The incidence of lower limb venous thrombosis in the observation group was lower than that in control group(P<0.05).In the clinical treatment of patients with lower limb hemiplegia after stroke,the use of air pressure wave combined with Xueshuantong and antiplatelet therapy can relieve hemiplegia symptoms,improve the therapeutic effect,optimize the lower limb function,improve the limb blood flow,and reduce the risk of thrombosis.
