奥替溴铵对肠易激综合征患者肠道微生态和肠道屏障功能的影响 被引量:15

Effect of otilonium bromide on intestinal microflora and intestinal barrier function in patients with irritable bowel syndrome

摘要 目的观察奥替溴铵对肠易激综合征(IBS)患者肠道微生态和肠道屏障功能的影响。方法将160例IBS患者按随机数字表法分为对照组和观察组各80例,对照组给予地衣芽胞杆菌胶囊、复方谷氨酰胺肠溶胶囊治疗,观察组在对照组基础上加服奥替溴铵。比较两组治疗效果。结果治疗后,观察组较对照组肠道大肠杆菌、粪肠球菌及血清二胺氧化酶(DAO)、D-乳酸、内毒素(ET)水平明显降低(P<0.05),乳酸杆菌、双歧杆菌、类杆菌水平及内脏初始感觉阈值、持续排便阈值、最大耐受阈值明显升高(P<0.05)。观察组较对照组症状严重程度各方面(腹痛程度、腹痛频率、腹胀程度、排便习惯满意度、对生活的影响)评分及总评分、匹兹睡眠质量量表(PSQI)评分明显降低,IBS生活质量评价量表(IBS-QOL)评分明显升高(P<0.05)。结论奥替溴铵可有效调节肠道微生态平衡,保护肠道屏障功能,改善内脏高敏性,有助于促进临床症状缓解,且安全性高。 To observe the effect of otilonium bromide on intestinal microflora and intestinal barrier function in patients with irritable bowel syndrome(IBS).A total of 160 IBS patients were randomly divided into the control or observation group,80 in each group.The control group was treated with bacillus licheniformis capsules and compound glutamin entersoluble capsules.The observation group was treated with otilonium bromide in addition to the control group treatment.The curative effect was compared between the two groups.After treatment,levels of Escherichia coli and Enterococcus faecalis in intestinal tract,and serum levels of DAO,D-lactic acid and endotoxin in the observation group were significantly lower than those in the control group(P<0.05),while levels of Lactobacillus,Bifidobacterium and Bacteroides,as well as the thresholds of visceral first sensation,continuous defecation and maximum tolerance were significantly higher than those in the control group(P<0.05).After treatment,scores of the degrees of symptoms such as abdominal pain,abdominal pain frequency,abdominal distension,satisfaction of defecation habits and effect on life and the total score,the Pittsburgh Sleep Quality Scale(PSQI)score in the observation group were significantly lower while IBS quality of life scale(IBS-QOL)score was significantly higher than the control group(P<0.05).Otilonium bromide can effectively regulate the intestinal microflora balance,protect the intestinal barrier function and improve the visceral hypersensitivity.These can help to alleviate the clinical symptoms.The safety is also high.
