摘要
目的评估超声和MRI对宫颈癌术前分期的准确性及核磁弥散序列在随访过程中的意义。方法经本院病理科证实的宫颈癌患者95例,术前均进行MRI和超声检查,以病理证实为金标准,对比超声和MRI对宫颈癌术前T和N分期的准确性。术后随访患者的复发情况,对比复发患者的病灶区与邻近正常组织的表观扩散系数(ADC)值情况,了解核磁弥散序列在宫颈癌患者术后随访过程中的意义。结果超声对T1的准确率高于MRI,MRI对T_2、T_3、T_4的准确率高于超声(P<0.05)。MRI对N_0和N_1检查阳性率高于超声(P<0.05)。宫颈癌术前病灶区ADC值明显低于正常组织(t=6.184,P=0.011)。宫颈癌术后复发患者的病灶区ADC值低于邻近正常组织(t=5.316,P=0.027)。结论 MRI对宫颈癌术前分期具有较高的准确性,能够为临床提供更为准确的信号,核磁弥散序列在宫颈癌术后随访过程中具有重要的临床指导意义。
To evaluate the accuracy of ultrasound and MRI in the preoperative staging of cervical cancer and the significance of the sequence in the follow-up process.A retrospective analysis of 95 cervical cancer patients confirmed by pathology was performed.All the patients received preoperative MRI and ultrasound examination.The accuracy of preoperative staging of T and N of ultrasonography and MRI was compared with pathological golden standard.Postoperative follow-up was performed for patients relapse,and ADC value of recurrent lesions area and adjacent normal tissues were compared to understand the significance of nuclear magnetic dispersion sequence in the follow-up process.The accuracy of ultrasonography for T1 staging was higher than MRI while the accuracy of MRI for T2,T3 and T4 were higher than ultrasonography(P<0.05).The positive rate of N0 and N1 of MRI was higher than ultrasound examination(P<0.05).The ADC value of the preoperative lesion of cervical cancer was significantly lower than that of normal tissue(t=6.184,P=0.011).The ADC values of cervical cancer lesion area of postoperative recurrence was lower than the adjacent normal tissues(t=5.316,P=0.027).The accuracy of MRI in preoperative staging of cervical cancer is high,that can provide more accurate signal.The clinical nuclear magnetic dispersion sequence in cervical cancer has important clinical significance in the process of postoperative follow-up.
