支气管镜下呼气末二氧化碳探查定位难治性气胸肺破裂口1例

摘要 自发性气胸是呼吸科常见病,多采用胸腔穿刺术或闭式引流术治疗。少数胸膜瘘口长期不愈大于两周的顽固性气胸,通常需采用胸膜硬化术、胸腔镜手术或开胸手术。基层医院呼吸科中慢性阻塞性肺疾病、肺尘埃沉着病(尘肺)等患者较多,肺功能较差、肺结构破坏严重易合并气胸,且合并气胸时瘘口难愈合,针对这些难治性气胸患者可选择支气管封堵术(SBO)。SBO是将肺破裂口所属的目标支气管封堵,使瘘口停止漏气加速其愈合。球囊探查具有简便、有效、经济及安全等优点,为提高引流支气管探查阳性率[1],笔者采用呼气末二氧化碳(EtCO2)联合球囊探查对胸膜瘘口引流支气管进行准确定位,并成功实施自体血瘘口封堵术治疗,现报道如下:患者男,45岁,因“胸闷1天”于2016年2月26日入院,X线胸片示左侧气胸,肺萎陷70%,诊断为“左侧自发性气胸”。
