董氏针法治疗急性踝关节扭伤临床疗效的随机对照研究

RCTs of Acute Ankle Sprain Treated by DONG's Acupuncture

目的:比较董氏针法与常规治疗对急性踝关节扭伤的治疗效果。方法:纳入符合标准的患者82例,最终受试者为79例,采用随机对照试验方法分为两组,实验组(39例):采用董氏针法,选取灵谷、大白和合谷,使用倒马针法及对应取穴,针刺得气后,使用动气针法,留针40 min,7天为1个疗程,共治疗3~4个疗程。对照组(40例):采用常规治疗方法,保护、休息、冰敷、加压、抬高患肢。治疗4周后,观察两组患者踝关节肿胀、踝关节AOFAS评分及VAS评分情况,同时比较两组疗效。结果:对于踝关节肿胀情况,第1天两组无显著差异(P>0.05),实验组在治疗第2天开始有效,而对照组自治疗第3天开始有效,比较有统计学意义(P<0.05);对于踝关节AOFAS评分及VAS评分比较,两组治疗有明显差异(P<0.05)。结论:两种治疗方法均不能立即减轻局部肿胀,但董氏针法能够及时减轻病痛降低VAS评分,加大扭伤踝关节的活动度,提高踝关节AOFAS评分,能够提高治愈率。

Objective:To compare the therapeutic effects of Dong's acupuncture and conventional treatment for acute ankle sprains.Methods:79 patients were randomly divided into the control group(n=40)and the treatment group(n=39).The observation group was treated with Dong s Daoma(inverted horse)acupuncture,points of Linggu,Dabai,Hegu and corresponding points were selected;after qi arrival,Dongqi manipulation was adopted.Each time the needles were remained for 40 minutes,seven days were taken as one course,the treatment was three to four courses in total.The control group was intervened with conventional treatment,such as protection,rest,ice compress,compression,and raising affected limbs.Ankle swelling,AOFAS score and VAS score of ankles were observed after four weeks of the treatment in the two groups,and the curative effect were compared between the two groups.Results:In terms of ankle swelling,there was no significant difference between the two groups on the first day(P>0.05).The effect started on the second day in the observation group,whereas it started on the third day in the control group,there was statistical significance between the two groups(P<0.05).There was also significant difference between the two groups in terms of ankle AOFAS score and VAS score(P<0.05).Conclusion:The both treatments can not immediately reduce the swelling,but Dong s acupuncture therapy can relieve pain and reduce VAS score in time,it can increase the activities of sprained ankle joint and improve the AOFAS score,it can also improve the cure rate.
