针药并用对2型糖尿病周围神经病变的疗效观察

Clinical Observation of Acupuncture Combined with Medication Treating Peripheral Neuropathy of Type 2 DM

目的:观察温针灸配合当归四逆汤治疗2型糖尿病周围神经病变的临床疗效。方法:筛选符合标准的患者60例,抽签法随机分为试验组和对照组,每组各30例,试验组给予温针灸合当归四逆汤治疗,对照组给予依帕司他和甲钴胺片口服,2个月为一疗程,统计分析患者的中医症候积分、toronto评分以及神经传导速度(mcv)、FBG、PBG、Hb A1c的变化。结果:试验组总有效率86.67%,对照组总有效率50%,试验组优于对照组(P<0.05)。肌电图检测试验组治疗后神经传导速度优于对照组,具有显著性意义(P<0.05)。toronto评分两组都有减低,试验组减分多于对照组(P<0.05)。治疗后中医症候评分两组差异较大(P<0.01)。试验前后两组血糖、糖化血红蛋白水平均稳定,两组治疗前后FBG、PBG、Hb A1c结果比较P>0.05。结论:温针灸配合当归四逆汤能减轻2型糖尿病周围神经病变患者的临床症状及体征,并提高神经传导速度。

Objective:To observe the clinical effect of needling-warming moxibustion combined with Danggui Sini decoction in the treatment of peripheral neuropathy of type 2 diabetes mellitus(DM).Methods:60 patients were randomly divided into the observation group(n=30)and the control group(n=30).The observation group was treated with needle-warming moxibustion combined with Danggui Sini decoction;whereas the control group was given Epalrestat and Mecobalamin tablets,the treatment was two months.The changes of TCM symptoms integral,Toronto ratings,nerve conduction velocity(NCV),FBG,PBG,HbA1c were observed.Results:The efficiency of the observation group was 86.67%,which was significantly higher than 50%of the control group(P<0.05).EMG showed that NCV of the observation group was superior to that of the control group after the treatment(P<0.05).Toronto rating was reduced in the two groups after the treatment,and the reduction of the observation group was more significant than that of the control group(P<0.05).There were significant difference between the two groups in terms of the TCM symptom integral after the treatment(P<0.01).There were no statistical significance between the two groups before and after the treatment in terms of FBG,PBG,and HbA1(P>0.05).Conclusion:The therapy of needling-warming moxibution combined with Danggui Sini decoction can alleviate the clinical symptoms and signs,as well as improve NCV in type 2 DM patients with peripheral neuropathy.
