关节镜下交叉韧带重建自体腱加异体腱联合移植与异体腱移植的术后疗效的差别

Arthroscopic Reconstruction of Cruciate Ligament with Autologous Tendon Plus Allogeneic Tendon Transplantation and Allogeneic Tendon Transplantation

目的对比分析关节镜下交叉韧带重建采用自体腱联合异体腱移植与异体腱移植术的疗效。方法采用随机、双盲、对照试验,方便选取该院2015年6月—2016年12月接诊的67膝前交叉韧带(ACL)损伤患者,入选患者均接受关节镜下交叉韧带重建术治疗,根据移植物的差异性分组,观察组(n=34)采用自体腱联合异体腱移植,对照组(n=33)采用异体腱移植,观察两组患者术后临床治疗效果。结果与对照组(2.33±1.02)cm比较,观察组髌上1 cm双侧大腿周径差值(1.43±0.15)cm更小,Lysholm评分(83.46±7.77)分、Tegner评分(90.15±6.46)分、IKDC评分(7.15±1.1)分显著高于对照组(71.73±4.34)分、(83.46±5.88)分、(6.49±0.97)分,差异有统计学意义(P<0.05)。结论关节镜下交叉韧带重建采用自体腱联合异体腱移植可取得较单独采用异体腱移植的手术效果更好,前者更利于关节功能的恢复。

Objective This paper tries to compare and analyze the clinical effects of joint transplantation of autogenous tendon and allogeneic tendon and transplantation of allogeneic tendon for arthroscopic reconstruction of cruciate ligament.Methods 67 patients with anterior cruciate ligament injury(ACL)and treated with arthroscopic cruciate ligament reconstruction in this hospital from June 2015 to December 2016 by the randomized,double-blind,controlled trial were grouped according to the difference of grafts.The observation group(n=34)was given the autogenous tendon with allograft of allogeneic tendon,while the control group(n=33)was given the allograft of allogeneic tendon to observe the postoperative effects of two groups.Results Compared with the control group of(2.33±1.02)cm,the difference of bilateral thigh circumference(1.43±0.15)cm in the patellar 1cm in the observation group was smaller.And the Lysholm score(83.46±7.77)points,Tegner score(90.15±6.46)points and IKDC score(7.15±1.1)points,which was significantly higher than those in the control group of(71.73±4.34)points,(83.46±5.88)points and(6.49±0.97)points,and the difference was statistically significant(P<0.05).Conclusion Joint transplantation of autogenous tendon and allogeneic tendon can achieve better surgical effect and recovery of joint function than the single use of allogeneic tendon in arthroscopic reconstruction of cruciate ligament.
