小剂量阿奇霉素在低呼出气一氧化氮(FeNO)测定值哮喘中的应用价值 被引量:1

摘要 目的探讨小剂量阿奇霉素在低呼出气一氧化氮(FeNO)测定值哮喘中的应用价值。方法该实验方便选取2015年6月—2017年6月期间在该院诊治的慢性中度持续期哮喘患者66例,随机分两组:A组(n=33):舒利迭(规格:250μg)吸人,2次/d,B组(n=33):除给予A组治疗外,加用阿奇霉素,口服,0.25 g,1次/d,每周连服3 d;连续治疗24周。观察FEV1/预计值,PEF变异率,哮喘控制测试(ACT)评分变化及药物不良反应。结果治疗后与治疗前比较,A组治疗后FEV1/预计值(73.23±4.98)%,ACT评分(19.94±2.56)%,PEF变异率(29.56±5.41)%较治疗前FEV1/预计值(69.05±3.45)%,ACT评分(16.64±2.19)分,PEF变异率(37.00±2.71)%有明显好转(P<0.05),B组治疗后FEV1/预计值(77.15±6.15)%,ACT评分(22.58±2.00)分,PEF变异率(26.95±5.01)%较治疗前FEV1/预计值(68.10±5.59)%,ACT评分(16.79±2.27)分,PEF变异率(37.14±3.96)%有显著改善(P<0.05),在B组治疗后与A组治疗后比较中,B组FEV1/预计值(77.15±6.15)%,ACT评分(22.58±2.00)分,PEF变异率(26.95±5.01)%与A组治疗后FEV1/预计值(73.23±4.98)%,ACT评分(19.94±2.56)分,PEF变异率(29.56±5.41)%比较,B组较A组改善更加显著(P<0.05)。结论对于低FeNO测定值哮喘患者,长期小剂量服用阿奇霉素,可提升FEV1/预计值,降低PEF变异率,有助于哮喘控制管理。 Objective To study the application value of small-dose azithromycin in the asthma with low expiratory air FeNO measurement value.Methods 66 cases of asthma patients with chronic and moderate duration in our hospital from June 2015 to June 2017 were convenient selected and randomly divided into groups with 33 cases in each,the group A used the seretide(specification:250μg),2 times/d,while the group B used the azithromycin on the basis of the group A,oral administration 0.25 g,1 times/d,for three days every week,and 24 weeks in total,and the FEV1/predication value,PEF variation rate,ACT score changes and adverse reactions of drugs were observed.Results The FEV1/predication value,ACT score and PEF variation rate in the group A after treatment were obviously improved compared with those before treatment,[(73.23±4.98)%,(19.94±2.56)points,(29.56±5.41)%vs(69.05±3.45)%,(16.64±2.19)points,(37.00±2.71)%](P<0.05),in the group B,the FEV1/predication value,ACT score and PEF variation rate after treatment were obviously improved compared with those before treatment,[(77.15±6.15)%,(22.58±2.00)points,(26.95±5.01)%vs(68.10±5.59)%,(16.79±2.27)points,(37.14±3.96)%]P(P<0.05),and the FEV1/predication value,ACT score and PEF variation rate in the group B after treatment were more obvious than those in the group A,[(77.15±6.15)%,(22.58±2.00)points,(26.95±5.01)%vs(73.23±4.98)%,(19.94±2.56)points,(29.56±5.41)%](P<0.05).Conclusion The long-term administration of small-dose azithromycin for asthma patients with low FeNO measurement value can improve the FEV1/predication value and reduce the PEF variation rate,and it contributes to asthma control management.
