多排螺旋CT双动脉期增强扫描对比剂浓度对肝癌检出率的影响分析

Analysis of the Influence of Multi-slice Spiral CT Double Contrast Enhanced Contrast Concentration on Detection Rate of Hepatocellular Carcinoma

目的探究肝癌患者在多排螺旋CT双动脉期增强扫描中,运用不同浓度对比剂在病灶检出率方面产生的影响。方法方便选取在2015年1月—2017年5月该院收治的肝癌患者110例,随机分组原则分为观察组和对照组,每组55例。两组进行多排螺旋CT双动脉期增强扫描。观察组注射370 mg I/m L(1.5 m L/kg)碘帕醇对比剂,对照组注射300 mg I/m L(1.85 m L/kg)。分析两组诊断敏感性情况。结果被证实的肝癌病灶270个,≤2 cm及>2 cm的肝癌病灶分别有105个和165个。在≤2 cm病灶的诊断中,观察组诊断敏感性较高(P<0.05),且在所有病灶中,观察组动脉早期、晚期诊断敏感性分别为74.81%、91.85%均高于对照组65.93%、82.96%(P<0.05);但在门脉期诊断中,其诊断敏感性差异无统计学意义(P>0.05)。结论在动脉早期和晚期诊断中,高浓密度对比剂能够使肝癌检出率提高,而对于小于或等于2 cm的肝癌病灶,其敏感性更高。

Objective This paper tries to investigate the effect of different concentration of contrast agent on the detection rate of hepatocellular carcinoma in multi-slice spiral CT double arterial phase enhanced scan.Methods 110 patients with hepatocellular carcinoma treated in the hospital from January 2015 to May 2017 were randomly divided into observation group and control group,with 55 cases in each group.The two groups were subjected to multi-slice spiral CT double arterial phase enhanced scan.The control group was injected with 370 mgI/mL(1.5 mL/kg)of iopamidol and 300 mg/mL(1.85 mL/kg).The diagnostic sensitivity of two groups was analyzed.Results There were confirmed liver cancer lesions of 270,less than or equal to 2 cm and greater than 2 cm of liver cancer lesions were 105 and 165 respectively.In the diagnosis of less than or equal to 2 cm lesions,the sensitivity of the observation group was 74.81%and 91.85%higher than that of the control group(P<0.05).In all the lesions,the sensitivity of the early and late diagnosis 65.93%,82.96%(P<0.05);but in the diagnosis of portal diagnosis,the diagnosis of sensitive data The difference was not significant(P>0.05).Conclusion In the early and late diagnosis of arteries,high concentration contrast agent can improve the detection rate of liver cancer,and less than or equal to 2 cm of liver cancer lesions,its sensitivity is higher.
