摘要
目的:探讨^(18)F-FDG PET/CT显像胰腺癌原发灶肿瘤代谢体积(metabolic tumor volume,MTV)、最大标准摄取值(maximum standardized uptake value,SUVmax)及分期之间的相关性。方法:回顾性分析2011年1月至2016年12月于本院行^(18)F-FDG PET/CT显像后诊断为胰腺癌的患者79例,以肿瘤原发灶SUVmax的40%为阈值来确定肿瘤边缘,由工作站自动算出肿瘤原发灶的MTV、SUVmax,将胰腺癌重新分期为Ⅰ期(Ⅰa、Ⅰb期合并)、Ⅱa期、Ⅱb期、Ⅲ期、Ⅳ期,用Spearman检验分析MTV、SUVmax以及分期之间的相关性。结果:MTV与胰腺癌分期、SUVmax有相关性(r=0.396、0.450,P<0.001),SUVmax与胰腺癌分期无明显相关性(r=0.195,P=0.08)。单因素方差分析不同分期的MTV有统计学意义(P=0.01)。非参数假设检验独立样本Kruskal-Wallis检验分析,不同分期的SUVmax无统计学意义(P>0.05)。结论:胰腺癌原发灶MTV与其分期、SUVmax有一定的相关性,与SUVmax显著相关。胰腺癌患者早期行PET/CT检查测得肿瘤原发灶MTV在一定程度上有助于胰腺癌的临床分期。
Objective:To explore the relationship between the metabolic tumor volume(MTV),maximum standardized uptake value(SUVmax),and stage of primary tumor on 18F-fluorodeoxyglucose(FDG)positron emission tomography/computed tomography(PET/CT)in pancreatic cancer.Methods:A retrospective analysis was performed on 79 patients diagnosed with pancreatic cancer by 18F-FDG PET/CT in our hospital from January 2011 to December 2016.The edge of primary tumor was defined using 40%of the SUVmax as the threshold value.The MTV and SUVmax of primary tumor were automatically calculated by the workstation.Pancreatic cancer was reclassified into stage I(combining Ia with Ib),IIa,IIb,III,and IV.The relationship between MTV,SUVmax,and tumor stage was analyzed using Spearman rank correlation.Results:Both tumor stage and SUVmax were positively correlated with MTV(r=0.396,P<0.001;r=0.450,P<0.001),while there was no significant correlation between SUVmax and tumor stage(r=0.195,P=0.08).One-way analysis of variance showed that there was a significant difference in MTV between different tumor stages(P=0.01).The Kruskal-Wallis nonparametric test showed that there was no significant difference in SUVmax between different tumor stages(P>0.05).Conclusion:The MTV of primary tumor of pancreatic cancer has a certain degree of correlation with tumor stage and a significant correlation with SUVmax.Early 18F-FDG PET/CT scan in patients with pancreatic cancer may help predict the tumor stage by determination of the MTV of primary tumor.
