CEA、CA19-9及CA72-4联合检测在胃癌诊断中的意义

Significance of combined detection of CEA, CA19-9 and CA72-4 in the diagnosis of gastric cancer

目的探讨肿瘤标志物CEA、CA19-9及CA72-4联合检测在胃癌诊断中的意义。方法选取2014年6月到2017年6月在我院经病理确诊的50例各期胃癌患者,以及同期在我院治疗的胃良性病变者50例和健康体检者50例作为研究对象,通过电化学发光分析法测定并分析所有研究对象血清中的CEA、CA19-9及CA72-4水平。结果胃癌组各期CEA、CA19-9及CA72-4的水平均高于胃良性病变组与健康组(P<0.05),且胃癌组Ⅲ、Ⅳ期的被检者CEA、CA19-9、CA72-4水平升高更明显(P<0.01)。胃癌组CEA阳性21例,CA19-9阳性23例,CA72-4阳性19例,三项联合检测阳性43例。胃良性病变组和健康组共100例,其中CEA阴性41例,CA19-9阴性38例,CA72-4阴性39例,三项联合检测阴性85例。三项联合检测对诊断胃癌的敏感性和特异性均明显高于单项检测,差异具有统计学意义(P<0.05)。结论 CEA、CA19-9及CA72-4在胃癌患者中高表达,且三项联合检测可显著提高临床诊断胃癌的敏感性和特异性,对胃癌的早期诊断有着重要的意义。

Objective To investigate the significance of combined detection of CEA,CA19-9 and CA72-4 in the diagnosis of gastric cancer.Methods Fifty patients with gastric cancer diagnosed by pathology from June 2014 to June 2017,50 cases of benign gastric lesions and 50 healthy subjects were selected as the subjects.The levels of CEA,CA19-9 and CA72-4 in serum of all the subjects were determined by electrochemiluminescence assay.Results The levels of CEA,CA19-9 and CA72-4 in gastric cancer group were higher than those in benign gastric lesions group and healthy group(P<0.05),and the improvement levels of CEA,CA19-9 and CA72 in stageⅢandⅣof gastric cancer group was more obvious(P<0.01).There were 21 cases of CEA positive,23 cases of CA19-9 positive,19 cases of CA72-4 positive in gastric cancer group and 43 cases of three joint positive tests.There were 100 cases of benign gastric lesions and healthy group,41 cases were CEA negative,38 cases were CA19-9 negative,39 cases were CA72-4 negative,and 85 cases were three joint negative tests.The sensitivity and specificity of the three joint tests in diagnosing gastric cancer were significantly higher than those in single test(P<0.05).Conclusion CEA,CA19-9 and CA72-4 are highly expressed in gastric cancer patients,and three joint tests can significantly improve the sensitivity and specificity of clinical diagnosis of gastric cancer,which is a great significance in the early diagnosis of gastric cancer.
