个性化护理干预对社区高血压患者自我管理的影响

Impact of individualized nursing intervention on self-management in community hypertensive patients

目的评价个性化护理干预对社区高血压患者自我管理的影响,为社区护理干预工作以及患者自我管理行为的养成提供参考。方法选择100例社区高血压患者作为试验对象,患者均接受个性化护理干预,比较护理干预前、后患者对高血压相关知识的知晓率、自我管理情况以及收缩压、舒张压水平。结果较干预前,干预6个月后患者对高血压相关知识的知晓率、自我管理率均明显提高(P<0.05)。干预后患者的收缩压、舒张压水平均低于干预前(P<0.05)。结论对于社区高血压患者而言,个性化护理干预的实施有利于提高其对高血压知识的知晓率、自我管理能力,降低其舒张压、收缩压水平,可在社区实施,以提高患者治疗预后,促进健康生活方式的养成。

Objective To evaluate the impact of individualized nursing intervention on community-based self-management of hypertensive patients,and to provide reference for community-based nursing intervention and the development of patients'self-management behaviors.Methods A total of 100 community-based hypertensive patients were selected as experimental objects.Individualized nursing interventions were conducted.The patient's awareness of hypertension related knowledge,self-management,systolic and diastolic blood pressure before and after nursing intervention were compared.Results Compared with before intervention,the awareness rate of hypertension-related knowledge and self-management increased significantly after 6 months after intervention(P<0.05).The systolic blood pressure and diastolic blood pressure of the patients after intervention were lower than those before intervention(P<0.05).Conclusion In community-based hypertensive patients,the implementation of individualized nursing intervention is conducive to the awareness of hypertensiverelated knowledge,improvement of self-management behavior and diastolic blood pressure,systolic blood pressure,which can be implemented in community,so as to promote patients'treatment prognosis and development of health life.
