直接抗人球蛋白试验阳性患者的输血疗效评估

Evaluation of transfusion efficacy in patients with direct anti globulin test

目的对直接抗人球蛋白试验(DAT)阳性患者的输血疗效及输血安全性进行评估,为该类患者寻求更加安全、有效的输血对策。方法选取20例在本科输血且DAT阳性的患者作为实验组,再选取40例同时期在本科输血且DAT阴性的常规输血患者作为对照组,对两组患者输血前、后的Hb、RBC及Hct进行比对。结果实验组患者输注红细胞后,Hb、RBC及Hct均有统计学意义(P<0.05);对照组患者输注红细胞后,Hb、RBC及Hct均有统计学意义(P<0.05);实验组与对照组的输血总有效率差异无统计学意义。结论 DAT阳性患者输血时常常难以找到完全相合的红细胞,而该类患者有时又可能存在输血无效,因此只在紧急情况下输注少量红细胞,不应该盲目的由于配血不相合而坚持不给予输血,也不应该由于患者贫血而盲目输血。对于非抢救患者应首先查找病因,这样可以提高输血有效率,减少输血不良反应的发生。

Objective To evaluate the efficacy and safety of transfusion in patients with direct anti globulin test(DAT)positive,and to seek safer and more effective transfusion strategies for these patients.Methods 20 patients with DAT positive transfusion in our department were selected as the experimental group,and 40 routine transfusion patients with DAT negative transfusion in our department were selected as the control group,Hb,RBC and Hct were compared before and after blood transfusion between the two groups.Results The experimental group of patients with transfusion of red blood cells,Hb,RBC and Hct had statistical significance(P<0.05);the control group of patients with transfusion of red blood cells,Hb,RBC and Hct had statistical significance(P<0.05);the experimental group and the control group the total efficiency of blood transfusion was not statistically difference.Conclusion DAT patients with positive blood transfusion is often difficult to find identical red blood cells,and these patients sometimes there may be blood transfusion is invalid,so only a small amount of red blood cell infusion in J emergency situations,should not blindly due to mismatched with blood and insist not to give blood,it should not be in patients with anemia and blind blood transfusion.For non salvage patients,we should first find the cause of the disease,which can improve the efficiency of blood transfusion and reduce the incidence of adverse reactions.
