双C疗法联合维生素D治疗高龄妊娠期糖尿病患者的效果评价

Effect evaluation of double C therapy combined with vitamin D in the treatment of elderly GDM patients

目的研究双C疗法联合维生素D治疗高龄妊娠期糖尿病患者的效果评价,为临床治疗提供依据。方法选择2015年5月~2017年5月我院收治的100例高龄GDM患者为研究对象,按照随机数表法平均分为实验组与对照组,每组各50例。对照组患者给予传统多次胰岛素皮下注射治疗,实验组给予双C疗法联合维生素D治疗,两组患者均干预治疗两周。观察比较两组患者治疗后血糖水平、妊娠以及新生儿变化情况。结果经治疗后,实验组患者的空腹血糖(FPG)、餐后2小时血糖(2h PG)和糖化血红蛋白(Hb A1c)水平均低于对照组,且其血糖达标时间明显比对照组短,差异有统计学意义(P<0.05)。实验组患者胎膜早破、羊水过多、妊娠期高血压疾病、早产儿、死胎和新生儿窒息的发生率均显著低于对照组(P<0.05);但两组患者的剖宫产、巨大儿和高胆红素血症的发生率相比较,差异无统计学意义(P>0.05)。结论双C疗法联合维生素D治疗GDM患者,可以更快更好地控制血糖水平,减少妊娠期间并发症的发生,提高新生儿预后,在临床上具有较高的应用价值。

Objective To study the effect of double C therapy combined with vitamin D in the treatment of elderly patients with gestational diabetes mellitus(GDM),so as to provide evidence for clinical treatment.Methods One hundred elderly patients with GDM in our hospital from May 2015 to May 2017 were selected as the research subjects.According to the random number table method,all them were divided into the experimental group and the control group,50 cases in each group.The patients of control group were treated with traditional multiple times subcutaneous insulin injection,while the patients of experimental group were given double C therapy combined with vitamin D treatment.The patients of two groups were all intervened for two weeks.Blood glucose levels,pregnancy and neonatal changes after treatment were compared between the two groups.Results After treatment,the level of fasting blood glucose(FPG),2 hours postprandial blood glucose(2H PG)and glycated hemoglobin(HbA1C)in the experimental group were lower than those in the control group,and the blood glucose level was shorter than the control group,and the difference was statistically significant(P<0.05).The incidence of premature rupture of membranes,excessive amniotic fluid,hypertension in pregnancy,premature infants,stillbirth and neonatal asphyxia were significantly lower in the experimental group than in the control group(P<0.05).However,there was no significant difference in the incidence of cesarean section,macrosomia and hyperbilirubinemia between the two groups(P>0.05).Conclusion Double C therapy combined with vitamin D in the treatment of GDM patients can control blood sugar faster and better,reduce the occurrence of complications during pregnancy,and improve the prognosis of the newborn,which is of high clinical value.
