全程疼痛照护配合穴位按摩对人工膝关节置换术后患者早期康复的影响观察

Observation on the effect of whole course pain care combined with acupoint massage on early rehabilitation of patients after total knee arthroplasty

目的观察分析全程疼痛照护配合穴位按摩对人工膝关节置换术患者早期康复的临床效果。方法选取2016年4月~2017年11月行人工膝关节置换术的住院患者48例,按照入院顺序单双数随机分为对照组和实验组,每组各24例,在指导患者术后功能锻炼基础上,对照组患者实施按需止痛,实验组给予全程疼痛照护和穴位按摩联合治疗,观察比较两组患者手术后HSS评分、膝关节活动度以及治疗满意度。结果与对照组相比,术后实验组HSS评分升高更为明显,VAS评分降低更为显著,两组差异有统计学意义(P<0.05);在膝关节活动度上,实验组在术后第1、4、8和15天测量值均显著大于对照组(P<0.05),实验组患者对术后护理满意度高达91.7%,对照组仅为79.2%,两组差异有统计学意义(P<0.05)。结论对人工膝关节置换术患者进行全程疼痛照护联合穴位按摩护理康复治疗,患者疼痛控制质量明显提升,对护理满意度显著提高,具有在临床上广泛应用的价值。

Objective To observe and analyze the clinical effect of whole-course pain care combined with acupoint massage on early rehabilitation of patients undergoing total knee arthroplasty.Methods From April 2016 to November 2017,48 inpatients undergoing total knee arthroplasty were selected,according to the order of admission,the patients were randomly divided into control group and experimental group,24 cases in each group.On that basis of guiding the postoperative functional exercise of the patient,the patients in the control group were given pain relief on demand,and the patients in the experimental group were given whole course pain care and acupuncture massage combined treatment.HSS score,knee joint activity and treatment satisfaction were observed and compared between the two groups.Results Compared with the control group,the HSS score of the experimental group increased more significantly,and the VAS score decreased more significantly than the control group,the difference between the two groups was statistically significant(P<0.05).On the knee joint activity,the measured values of the experimental group on the 1st,4th,8th and 15th day after operation were significantly higher than those of the control group(P<0.05).The satisfaction rate of the patients in the experimental group was 91.7%after operation,while that in the control group was only 79.2%.the difference between the two groups was statistically significant(P<0.05).Conclusion Total pain care combined with acupoint massage nursing rehabilitation treatment for patients undergoing total knee arthroplasty can significantly improve the quality of pain control and the nursing satisfaction,which has wide clinical application value.
