老年心血管病合并焦虑抑郁症患者心律失常的动态心电图分析

Dynamic electrocardiogram analysis of arrhythmia in elderly patients with cardiovascular disease combined with anxiety and depression

目的观察动态心电图监测下老年心血管病合并焦虑抑郁症患者心律失常的发生情况,分析其与心律失常发生的关系。方法 360例老年心血管病患者,按照患者是否合并焦虑抑郁症分为观察组(合并焦虑抑郁症)和对照组(未合并焦虑抑郁症),各180例。所有患者均进行动态心电图监测,比较两组患者的心理状态和心律失常检出情况。结果观察组抑郁自评量表(SDS)、焦虑自评量表(SAS)评分为(53.49±5.24)、(52.77±4.80)分,均显著高于对照组的(32.55±4.17)、(35.61±3.94)分,差异具有统计学意义(P<0.05)。观察组房室早搏、室性早搏、心房纤颤、短阵房性心动过速和房室传导阻滞的检出率均高于对照组,差异具有统计学意义(P<0.05)。结论动态心电图监测下老年心血管病合并焦虑抑郁症患者心律失常检出率较高,焦虑抑郁症与老年心血管病患者心律失常的发生密切相关。

Objective To observe the occurrence of arrhythmia in elderly patients with cardiovascular disease and anxiety and depression under dynamic electrocardiogram monitoring,and analyze its relationship with the occurrence of arrhythmia.Methods A total of 360 elderly patients with cardiovascular disease were divided by whether complicated with anxiety and depression into observation group(with anxiety and depression)and control group(without anxiety and depression),with 180 cases in each group.All patients received dynamic electrocardiogram monitoring,and the psychological state and detection of arrhythmia between the two groups was compared.Results The observation group had self-rating depression scale(SDS)and self-rating anxiety scale(SAS)score respectively as(53.49±5.24)and(52.77±4.80)points,which were all obviously higher than(32.55±4.17)and(35.61±3.94)points in the control group,and the difference was statistically significant(P<0.05).The observation group had higher detection rate of atrial premature beats,ventricular premature beats,atrial fibrillation,paroxysmal atrial tachycardia and atrioventricular block than the control group,and their difference was statistically significant(P<0.05).Conclusion The detection rate of arrhythmia in elderly patients with cardiovascular disease combined with anxiety and depression is high under dynamic electrocardiogram monitoring.Anxiety and depression is closely related to the occurrence of arrhythmia in elderly patients with cardiovascular disease.
