自动痔疮套扎术和吻合器痔上黏膜环切术治疗内痔为主环状混合痔的效果观察

Effect of RPH and PPH for treatment of mainly mixed hemorrhoids

目的比较自动痔疮套扎术(RPH)和吻合器痔上黏膜环切术(PPH)治疗内痔为主环状混合痔的效果。方法选取2014-06—2016-06间东莞市长安医院收治的80例内痔为主的环状混合痔患者。将采用RPH治疗的患者作为观察组,将采用PPH治疗的患者作为对照组,各40例。比较2组的治疗效果。结果 (1)2组患者的愈合时间及疼痛、尿潴留、肛缘水肿、急便感、随访1 a肛门溢液的发生率及总有效率比较,差异无统计学意义(P>0.05)。(2)观察组的手术时间、术中出血量、术后出血率及随访1 a痔核脱垂、便血及急便感的发生率均优于对照组,差异有统计学意义(P<0.05)。结论 RPH和PPH均可取得良好的治疗效果,但RPH操作简单,且远期疗效良好。

Objective To study the RPH and PPH treating hemorrhoids mainly circumferential mixed hemorrhoids effect.Methods In our hospital from June 2014 to June 2016 were mainly internal circumferential mixed hemorrhoids 80 cases,and the clinical data were retrospectively analyzed.The 40 patients were treated with RPH as the observation group,40 patients were treated with PPH as control group,the treatment effect of two groups were observed and compared.Results The observation group and the control group the total efficiency 100%,can achieve clinical cure,no significant difference;compared with the control group,the postoperative bleeding rate of the observation group was lower(P<0.05),there was no significant difference in the incidence of pain,urinary retention,anal edge edema and the sensation of urgency(P>0.05);compared with the control group,the postoperative bleeding rate of the observation group was lower(P<0.05),there was no significant difference in the incidence of pain,urinary retention,anal edge edema and the sensation of urgency(P>0.05);1 year follow-up,compared with the control group,the observation group of hemorrhoids prolapse,hematochezia and fecal urgency have the lower incidence(P<0.05),the incidence rate of anus overflow liquid had no significant difference(P>0.05).Conclusion RPH and PPH have achieved good therapeutic effect,but RPH is simple and has better long-term effect.It is suitable for clinical promotion.
