127例肾嫌色细胞癌免疫组化分析

Immunohistochemical analysis of 127 cases of renal chromophobe cell

目的研究肾嫌色细胞癌(Renal chromophobe cell carcinom)的免疫组化特点,并从病理学角度探讨诊断肾嫌色细胞癌的分子标记物特点,以提高肾嫌色细胞癌的诊治水平。方法回顾性分析郑州大学第一附属医院收治的127例病理确诊为肾嫌色细胞癌的病理资料,分析其免疫组化特点,并复习相关文献。结果 127例病理确诊为肾嫌色细胞癌的免疫组化表达如下:S-100(11/98,11.2%),CK(111/113,98.2%),CK7(84/108,77.8%),CD10(52/109,47.7),CD117(105/111,94.6%),PAX-2(30/109,27.5%),PAX-8(79/96,82.3%),Vim(26/103,25.2%),EMA(103/104,99.0%),RCC(22/98,22.4%),Ki-67(6/111,5.4%),E-caderin(94/100,94.0%)。结论免疫标志物CK,CK7,EMA,E-caderin阳性及Vim,S-100阴性是肾嫌色细胞癌诊断的良好标记物。S-100A1的阴性表现还预示着肾嫌色细胞癌可能预后较好。CD117在肾嫌色细胞癌的细胞膜上的过表达可能是激酶抑制剂的潜在靶标,不过疗效仍需更多的研究证实。

Objective To study the immunohistochemical characteristics of Renal chromophobe cell carcinomics and to explore the molecular markers of chromophobe cell carcinoma in order to improve the diagnosis and treatment of chromophobe cell carcinoma.Methods Totally 127 cases of pathology diagnosed as renal chromophobe cell carcinoma were retrospectively analyzed.The characteristics of immunohistochemistry were analyzed and the related literatures were reviewed.Results The expression of immunohistochemistry were detected by:S-100(11/98,11.2%),CK(111/113,98.2%),CK7(84/108,77.8%)by.CD10(52/109,47.7),CD117(105/111,94.6%),PAX-2(30/109,27.5%),PAX-8(79/96,82.3%),Vim(26/103,(72/100,94.0%),RCC(22/98,22.4%),Ki-67(6/111,5.4%),E-caderin(94/100,94.0%).Conclusion The positive of CK,CK7,EMA,E-caderin markers and negative expression of Vim and S-100 are good markers for the diagnosis of chromophobe cells.The negative expression of S-100A1 also indicates that the prognosis of chromosomal cancer may be better.The overexpression of CD117 on the cell membrane of the chromophobe cell may be a potential target of kinase inhibitors,but the results still need more studies.
