InterTan髓内钉股骨距替代型髋关节置换在高龄股骨转子间骨折中的应用

Application of InterTan Intramedullary Nail and Hip Replacement in Elderly Patients with Intertrochanteric Fracture

目的:研究InterTan髓内钉和股骨距替代型髋关节置换在治疗高龄股骨转子间骨折的不同疗效。方法:选择2013年5月到2016年5月就医的股骨转子间骨折患者的100例,根据手术方式分为观察组和对照组。观察组50例,为股骨距替代型髋关节置换组,对照组50例,为InterTan髓内钉组;观察并比较两组患者之间的髋关节Harris评分、日常生活能力评分(ADL)、并发症发生率、手术时间、术中出血量、住院时间及术后开始下地时间。结果:两组患者Harris评分、ADL评分与并发症发生率差异无统计学意义(P>0.05)。观察组的手术时间、术中出血量、住院时间均高于对照组,两组间差异有统计学意义(P<0.05)。观察组下地活动时间早于对照组,两组间差异有统计学意义(P<0.05)。结论:股骨距替代型髋关节置可以缩短患者术后开始下地活动的时间,但是InterTan髓内钉治疗可以缩短患者的手术时间并减少术中出血量。

Objective:To study the different therapeutic effects of InterTan intramedullary nail and replacement hip replacement in the treatment of intertrochanteric fractures of the aged femur.Methods:100 cases of intertrochanteric fracture of the femur in our hospital from May 2013 to May 2016 were selected and divided into the observation group and the control group according to the operation mode.50 in the observation group were the replacement hip joint replacement group and 50 in the control group,which was InterTan intramedullary nail group.The Harris scores,activities of daily living(ADL),complication rate,operative time,intraoperative blood loss,hospitalization time and the time of postoperative onset between the two groups were observed and compared.Results:There was no significant difference in the Harris score,the ADL score and the incidence of complications in the two groups(P>0.05).The time of operation,the amount of bleeding and the time of hospitalization in the observation group were all higher than those of the control group,and there was a significant difference between the two groups(P<0.05).The activity time of the observation group was earlier than that of the control group,and there was a significant difference between the two groups(P<0.05).Conclusion:The replacement of hip joint with hip joint can shorten the time of postoperative activity,but Inter Tan intramedullary nail can shorten operation time and reduce intraoperative blood loss.
