高分辨磁共振成像在颅内动脉夹层的应用进展 被引量:4

摘要 颅内动脉夹层(intracranial arterial dissection,IAD)是指各种原因使血液成分通过破损的颅内动脉内膜进入血管壁,导致血管壁间剥离分层形成血肿,或颅内动脉壁内自发性血肿,造成血管狭窄、闭塞或破裂的一种疾病。如果形成瘤样突起,则称为颅内夹层动脉瘤(intracranial dissecting aneurysm,IDA)[1]。IAD是儿童和中青年脑卒中的重要原因之一,亚洲人发病率较欧美人高,男性多于女性,发生的平均年龄为50.4岁,后循环比前循环常见,椎动脉V4段是最常见的部位[2]。后循环颅内动脉夹层的主要临床症状是蛛网膜下腔出血(subarachnoid hemorrhage,SAH)、头痛、颈痛,而前循环颅内动脉夹层更常出现局部缺血症状[3]。IAD的病因不明,诱发因素包括高血压、口服避孕药、偏头痛或近期感染史;与IAD相关的疾病包括梅毒,多囊肾和结节性多动脉炎[]。近年来,由于成像技术的进步以及临床对颅内动脉疾病的认识程度加深,颅内动脉夹层(IAD)诊断率大幅提高。
