宫颈癌患者三维能量多普勒超声下血流情况与HPV分型的相关性研究 被引量:4

摘要 目的探讨高危型人乳头瘤病毒(HPV)16/18感染与其他分型感染的宫颈癌患者病变血流、微血管密度(MVD)及血管内皮生长因子(VEGF)的关系。方法选择住院宫颈癌患者共98例,采用原位PCR杂交和免疫组织化学法检测HPV16/18感染,根据检测结果将病例分为3组:仅HPV16阳性32例为HPV16组;仅HPV18阳性25例为HPV18组;HPV16和18均阳性18例为HPV16+18组;两者皆阴性、且低危型HPV阳性为对照组共23例,术前前各组行经阴道3D-CPA检测,判断计算血管指数(VI)和血管分级。术后病理标本进行免疫组化染色检测MVD及VEGF表达水平,分析各组各指标的相关性。结果 4组VEGF阳性率差异有统计学意义(P<0.05),其中对照组明显低于HPV16组、HPV18组及HPV16+18组(均P<0.05)。所有病例无0级血流,对照组I级比例明显高于HPV阳性各组(均P<0.05)。对照组III级比例明显低于HPV阳性各组(均P<0.05),II级血流比例比较差异均无统计学意义(均P>0.05)。4组MVD值和VI值差异均有统计学意义(均P<0.05),其中对照MVD值和VI值明显低于HPV16组、HPV18组和HPV16+18组(均P<0.05)。结论 HPV16/18阳性宫颈癌患者MVD与VEGF均高于低危型感染患者,三维超声显示血流更密集。
