60例胎儿中枢神经系统畸形产前MRI检查的诊断效果分析 被引量:7

摘要 目的探讨胎儿中枢神经系统畸形产前MRI检查的诊断价值。方法收集医院收治的60例经产前超声(US)检查提示胎儿中枢神经系统畸形的单胎孕妇,均于US检查后2 d内接受磁共振成像技术(MRI)检查,以产后诊断、引产尸检等随访结果作为诊断金标准,对比分析产前MRI检查对胎儿中枢神经系统畸形的诊断效果。结果本组60例随访结果为:单纯侧脑室增宽17例,胼胝体缺如14例,脑积水10例,Dandy-Walker畸形11例,蛛网膜囊肿6例和平滑脑2例。产前US检出率为68.33%(41/60),漏诊3例,误诊16例;MRI检出率86.67%(52/60),1例误诊,7例漏诊。产前MRI正确检出率高于US(P<0.05)。结论产前MRI检查可作为胎儿中枢神经系统畸形常规US检查的验证、补充手段,帮助优化诊断效果。 Objective To explore the diagnostic value of antenatal MRI examination for any malfor^nations in the central nervous system of fetuses.MeAods A total of 60 single-fetus pregnant women indicated with malfomiations in central nervous system of the fetuses by prenatal ultrasonic examination received the MRI examination within two days after the ultrasonic examination.Postpartum diagnosis,induced labor autopsy and other follow-up results were taken as the diagnostic criteria to compare and analyze the diagnostic effect of antenatal MRI examination for malformations in the central nervous system of fetuses.Rults The follow-up results of the 60 fetus cases were listed as follows:17 cases indicated simple fetal cerebral ventriculomegaly,14 cases indicated agenesis of corpus callosum,10 cases indicated hydrocephalus,11 cases indicated Dandy-Walker malformation,six cases indicated arachnoid cyst and two cases indicated smooth brain.The antenatal US detection rate was 68.33%(41/60).There were three missed diagnoses and 16 misdiagnoses;the MRI detection rate was 86.67%(52/60),including one misdiagnosis and seven missed diagnoses.The correct antenatal detection rate of MRI was higher than that of the US method(P<0.05).Conclusion Antenatal MRI examination can be applied as a validating and supplemental means for the conventional US examination for malformations in the central nervous system of fetuses,which can help to improve the effect of diagnosis.
