摘要
目的研究全甲状腺切除术在治疗双侧甲状腺癌中的应用疗效。方法选取本院2011年1月—2012年6月收治的42例患有双侧甲状腺癌并进行全甲状腺切除术的患者作为研究对象。结果 42例患者中均没有出现围手术期死亡情况,颈部淋巴结转移阳性数平均每例(6.37±0.63)个,14例患者双侧淋巴结均为阳性,所占比例为33.33%,20例患者单侧淋巴结阳性,所占比例为47.62%。在术后并发症方面,1例患者出现饮水呛咳症状,5例患者出现收治发麻或四肢轻微抽搐的现象,1例患者出现顽固性低钙血症症状,1例患者出现淋巴瘘症状。经过1年的电话随访,所有患者无声音嘶哑病例,且无患者术后死亡,无肿瘤复发现象。结论采用全甲状腺切除术治疗双侧甲状腺癌具有较好的治疗效果,能够保证手术的安全性,降低并发症的发生率,结合患者个人实际情况制定综合治疗方案,由于提高患者的生活质量,使患者快速恢复健康,具有一定的临床应用价值。
Objective To study the application and analysis of total thyroidectomy in the treatment of bilateral thyroid carcinoma.Methods 42 patients with bilateral thyroid cancer and total thyroidectomy from January 2011 to June 2012 were selected as the research subjects.Results There were no perioperative deaths in 42 patients.The number of positive cervical lymph nodes was(6.37±0.63)in each case,14 patients were positive for bilateral lymph nodes,the proportion was 33.33%,20 patients were positive for unilateral lymph nodes,accounting for 47.62%.In the postoperative complication,1 patient appeared hydroposia symptoms,the phenomenon occurred in 5 patients were slight numbness or twitching limbs,1 case of patients with intractable hypocalcemia symptoms,1 case of patients with lymphatic fistula symptom.After 1 year of telephone follow-up,all patients had no hoarse cases,and no postoperative death and no recurrence of the tumor.Conclusion The total thyroidectomy has a good therapeutic effect for the treatment of bilateral thyroid carcinoma,to ensure the safety of the operation,reduce the incidence of complications,making comprehensive treatment combined with individual actual situation,due to improve the life quality of patients,so that patients recover quickly.
