2060例乙酰半胱氨酸注射液的用药评价

Drug use evaluation of 2060 cases of acetylcysteine injections

目的:了解我院乙酰半胱氨酸注射液的使用情况,探讨其不良反应/事件(ADR/ADE)发生的规律和特点,为临床合理用药提供参考。方法:回顾性分析我院2016年6月–2017年8月2060例使用乙酰半胱氨酸注射液的病例,对适应证、疗程、给药剂量、给药频次、给药途径、溶媒、溶媒量、配伍禁忌、滴注速度及36例发生ADR/ADE的病例进行统计分析。结果:2060例使用乙酰半胱氨酸注射液的病例中,超说明书用药位列前三位的分别是溶媒选择不适宜(66.96%)、给药剂量不适宜(18.10%)、溶媒量不适宜(11.99%);36例ADR/ADE病例中,引起ADR/ADE的因素有未标注滴速(100.00%)、溶媒及溶媒剂量不适宜(69.44%)、无适应证用药(11.11%)。结论:应严格按照药品说明书规定合理使用乙酰半胱氨酸注射液,药品的不合理使用是导致ADR/ADE发生的主要原因。

Objective:To analyze the application of acetylcysteine injection in our hospital and summarize the characteristics of ADR/ADE associated with acetylcysteine injection,in order to provide references for rational drug use.Methods:The clinical data of the patients who were treated with acetylcysteine injection in our hospital from June 2016 to August 2017 were collected and analyzed retrospectively in terms of the indication,the course of treatment,dosages,frequency of administration,route of medication,type and amount of solvent,incompatibility and drip speed.A total of 36 cases of ADR/ADE associated with acetylcysteine injection were also analyzed.Results:Among the 2060 cases,the top three reasons of off-label drug use were unsuitable solvent(66.96%),inappropriate dosage(18.10%),unsuitable amount of solvent choice(11.99%).Among the 36 ADR/ADE cases,the factors resulted in ADR/ADE were unlabeled drop rate(100.00%),unsuitable type and amount of solvent(69.44%)and unindicated drug use(11.11%).Conclusion:Acetylcysteine injection should be strictly used in accordance with drug instruction,the irrational use of drugs was the main cause of ADR/ADE.
