腹腔镜微创术治疗老年直肠癌的疗效观察

Observation of curative effects of laparoscopic minimally-invasive surgery for elderly patients with rectal cancer

目的:观察腹腔镜微创术治疗老年直肠癌的疗效。方法:选取60例老年直肠癌患者为研究对象,按照入院先后顺序将其分为观察组与对照组各30例,对照组采用传统开放式手术治疗,观察组采用腹腔镜微创术治疗,对比两组并发症发生率、手术时间、住院时间、总有效率、临床获益率、体重、Karnofsky评分和排尿功能。结果:观察组术后发生并发症发生率为3.33%(1/30),明显低于对照组的16.67%(5/30);观察组手术时间和住院时间分别为(72.05±10.87)min和(7.64±7.68)d,均明显短于对照组的(125.99±21.54)min和(12.47±2.73)d;观察组总有效率和临床获益率分别为33.33%和76.67%,均明显高于对照组的16.67%和50%;治疗2周后观察组体重与Karnofsky评分均明显高于对照组;观察组排尿功能0级和Ⅰ级的患者明显多于对照组,组间比较差异均有统计学意义(P<0.05)。结论:腹腔镜微创术治疗老年直肠癌患者的疗效优于传统开放式手术治疗疗效。

Objective:To observe effects of laparoscopic minimally-invasive surgery in treatment of elderly patients with rectal cancer.Methods:60 cases of elderly patients with rectal cancer were selected as the research objects and divided into observation group and control group with 30 cases in each group according to the admission order.The control group used the traditional open surgery treatment,while the observation group was treated with laparoscopic minimally-invasive surgery.The complication rates,operation time,hospitalization time,total effective rates,clinical benefit rates,body weights,Karnofsky scores and urination function of the two groups were compared.Results:The postoperative complication rate of the observation group(3.33%,1/30)was significantly lower than that of the control group,and the operation time and hospitalization time(72.05±10.87 min,7.64±7.68 d)were obviously shorter than those of the control group(125.99±21.54 min,12.47±2.73 d).The total effective rate and clinical benefit rate of the observation group(33.33%and 76.67%)was significantly higher than those of the control group(16.67%and 50%)(P<0.05).2 weeks after the treatment,the weight and Karnofsky scores of the observation group was significantly higher than those of the control group.The cases with the urination function 0 and I levels in the observation group were significantly more than those of the control group,and the difference was statistically significant(P<0.05).Conclusions:Laparoscopic minimally-invasive surgery in the treatment of the elderly patients with rectal cancer is more effective than traditional open surgery.
