盆底肌锻炼对膀胱癌根治回肠原位新膀胱术后患者健康相关生活质量的影响

Effects of pelvic floor muscle training on health related quality of life of patients with construction of orthotopic ileal neobladder after radical cystectomy for bladder cancer

目的探讨盆底肌锻炼对膀胱癌根治回肠原位新膀胱术后患者健康相关生活质量中的影响。方法行膀胱癌根治回肠原位新膀胱术患者46例分为对照组和观察组,各23例。对照组常规行尿意训练及腹压排尿法,观察组在对照组基础上给于Kegel运动锻炼盆底肌肌力。对比2组患者术后健康相关生活质量。结果实施干预后,观察组患者在功能状况和膀胱癌模块方面评分优于对照组(P<0.05);观察组膀胱癌特异性量表(FACT-BL)总分高于对照组(P<0.05);观察组患者对护理效果、训练效果及护理工作总体评价结果优于对照组(P<0.05)。结论 Kegel运动能够促进膀胱癌根治回肠原位新膀胱术后新膀胱的功能恢复,使患者恢复正常排尿功能,改善患者生活质量。

Objective To investigate the pelvic floor muscle training(Kegel exercise)on health related quality of life of patients with construction of orthotopic ileal neobladder after radical cystectomy for bladder cancer.Methods Forty-six patients undergoing construction of orthotopic ileal neobladder after radical cystectomy for bladder cancer were enrolled in this study.Patients were divided into the control group(n=23)and the observation group(n=23).Patients of both two groups were conventional exercise on micturition desire and abdominal pressure for urination,and those in the observation group were given Kegel exercise.The health related quality of life of patients was evaluated by Fuctional Assessment of Cancer Therapy-Bladder(FACT-BL).Results Patients in the observation group achieved higher scores of functional well being and bladder-specific subscale compared with those in the control group(P<0.05)after nursing intervention.The Total FACT-BL score in the observation group was higher than that of controls(P<0.05).More patients of the observation group were satisfied with the nursing efficacy,training efficacy and overall nursing service compared with those of the control group(P<0.05).Conclusion Kegel exercise is potentially effective to improve the functional rehabilitation of neobladder,so as to improve the health related quality of life of patients.
