手术联合外固定治疗老年性肘关节骨折脱位的临床疗效分析

The Clinical Effect of Surgical Combined External Fixation in the Treatment of Senile Elbow Fracture Dislocation

目的目的:探究手术联合外固定治疗老年性肘关节骨折脱位的临床疗效分析。方法分析本院7例老年性肘关节骨折脱位患者,对其中6例三联征损伤进行挠骨头骨折的Mason分类。其中Ⅰ型1例、Ⅱ型3例、Ⅲ型2例。按照Regan-Morey对尺骨冠突骨折进行分类,其中Ⅰ型2例、Ⅱ型2例、Ⅲ型1例。根据患者尺骨冠状骨折的类型对其采用克氏针或是螺钉钢板内固定,并修补侧副韧带。术后患者前臂屈肘90°并旋转立位石膏三周。三周后可将石膏去除,改用肘关节几何辅助器具展开锻炼。结果在6个月的随访过程中,6例患者均有较好的骨折愈合度,且肘关节恢复稳固,没有痛感。经过肘关节Mayo评分可知优3例、良3例。结论老年患者由于骨质疏松易骨折脱位,手术中很难对其固定。应在辅助手段下对其进行基本的功能修复。

Objective To explore the clinical curative effect analysis of fracture and dislocation combined with external fixation for the treatment of senile elbow joint.Methods analysis of our hospital 7 cases of elderly patients with fracture and dislocation of elbow joint,of which 6 cases of triad injury by Mason classification of radial head fractures.2 cases of type I,3 cases of type II,1 type 3.According to the Regan-Morey of ulna coronoid process fractures were classified,including 2 cases of type I,3 cases of type II,3 cases of type III patients.According to the type of the patient s ulnar coronal fracture,it was used to fix it with kirschner wire or screw plate,and repair the collateral ligament.After surgery,the forearm flexion was 90 degrees and rotated for three weeks.After three weeks,the gypsum can be removed,and the elbow joint geometry accessory can be used for exercise.Results During the follow-up period of 20 months,6 patients showed good fracture healing and recovery of elbow stability,which was no pain.The elbow joint Mayo score showed there were 3 excellent cases,3 good cases.conclusion:elderly patients with osteoporosis due to fracture dislocation,it is difficult to fixation.Then repair the basic functions of the auxiliary means.
