前置胎盘剖宫产患者产后出血危险因素分析

Risk factors a nalysis for postpartum hemorrhage in caesarean section patients due to placenta previa

目的探讨前置胎盘剖宫产患者产后出血发生危险因素。方法选取2016年3月~2017年7月我院收治的前置胎盘住院分娩患者57例临床资料行回顾性分析,根据患者剖宫产后是否发生产后出血分为产后出血组和未产后出血组,收集两组患者一般临床资料及可疑危险因素,采用统计学软件进行单因素和多因素Logistic回归分析,探究影响前置胎盘剖宫产产后出血发生危险因素。结果 57例前置胎盘剖宫产患者产后出血发生17例,发生率为29.82%。两组患者在产次、前置胎盘附着部位、前置胎盘类型、产前出血、初次产前出血孕龄、胎盘植入方面的比较,差异有统计学意义(P<0.05);多因素分析得出,前置胎盘附着前壁、中央型前置胎盘、产前出血、胎盘植入为前置胎盘剖宫产患者产后出血发生危险因素前。结论影响前置胎盘剖宫产产后出血危险因素较多,临床中应根据相关危险因素针对性进行预防和干预,以降低前置胎盘剖宫产产后出血发生率。

Objective To investigate the risk factors of postpartum hemorrhage in caesarean section patients due to placenta previa.Methods The clinical data of 57 cases of placenta previa hospitalized in our hospital from March 2016 to July 2017 were retrospectively analyzed.according to whether postpartum hemorrhage occurred after cesarean section,the patients were divided into postpartum hemorrhage group and non postpartum hemorrhage group.The general clinical data and suspected risk factors of the two groups were collected,and the univariate and multivariate logistic regression analysis was used to explore the risk factors of postpartum hemorrhage after cesarean section of placenta previa.Results There were 17 cases of postpartum hemorrhage in 57 cases of placenta previa cesarean section,the incidence rate was 29.82%.There were statistically significant differences between the two groups in the delivery time,placenta previa attachment site,placenta previa type,prenatal hemorrhage,gestational age of the first prenatal hemorrhage and placenta accreta(P<0.05).Multivariate analysis showed that placenta previa attached to the anterior wall,central placenta previa,prenatal hemorrhage,placenta implantation were the risk factors of postpartum hemorrhage in patients with placenta previa cesarean section.Conclusion There are many risk factors influencing on postpartum hemorrhage after cesarean section due in placenta previa.Targeted on the relevant risk factors,prevention and intervention should be given in order to reduce the incidence of cesarean section postpartum hemorrhage in placenta previa.
