双黄连粉针剂联合阿米卡星与头孢哌酮/舒巴坦对广泛耐药鲍曼不动杆菌的体外抑菌效果

In Vitro Antibacterial Study of Shuanghuanglian Combined with Amikacin and Cefoperazone/Sulbactam for Extensively Drug Resistant Acinetobacter Baumannii

目的探讨双黄连粉针剂联合阿米卡星与头孢哌酮/舒巴坦对广泛耐药鲍曼不动杆菌的体外抑菌效果。方法应用棋盘法设计,肉汤稀释法测定不同浓度组合双黄连与阿米卡星、头孢哌酮/舒巴坦对30株临床分离广泛耐药鲍曼不动杆菌的最低抑菌浓度,以部分抑菌浓度指数(FIC)为指标判定中西药联合抑菌作用。结果双黄连联合阿米卡星FIC结果显示:FIC≤0.5为0%,FIC>0.5~1为16.7%,FIC>1~2为70.0%,FIC>2为13.3%;双黄连联合头孢哌酮/舒巴坦FIC结果显示:FIC≤0.5为23.3%,FIC>0.5~1为73.3%,FIC>1~2为3.3%,FIC>2为0%。结论双黄连联合阿米卡星对广泛耐药鲍曼不动杆菌多数表现为无关作用和拮抗作用,体外联合抑菌作用较差;而双黄连与头孢哌酮/舒巴坦联合表现出的协同作用和相加作用比例较高,对广泛耐药鲍曼不动杆菌的体外联合抑菌作用较好。

Objective To discuss the effect of shuanghuanglian powder injection combined with amikacin and cefoperazone/sulbactam on extensively drug resistant acinetobacter baumannii in vitro.Methods Chessboard method and broth dilution method were used to determine the minimal inhibitory concentration of different concentrations of shuanghuanglian powder injection and amikacin,shuanghuanglian powder injection and cefoperazone/sulbactam in 30 strains of the clinical extensively drug resistant acinetobacter baumannii,the fractional inhibitory concentration(FIC)index was calculated to determine the combined effect of the antibiotics.Results FIC index results of shuanghuanglian combined amikacin showed:FIC≤0.5 was 0%,FIC>0.5-1 was 16.7%,FIC>1-2 was 70.0%,FIC>2 was 13.3%,FIC index results of shuanghuanglian combined cefoperazone/sulbactam showed:FIC≤0.5 was 23.3%,FIC>0.5-1 was 73.3%,FIC>1-2 was 3.3%,FIC>2 was 0%.Conclusion The combined effect of shuanghuanglian and amikacin is most irrelevant and antagonistic effect,the combined antibacterial effect is not ideal.The combined effect of shuanghuanglian and cefoperazone/sulbactam is most synergistic and additive effect,so the combined antibacterial effect is better.
