新生儿感染性肺炎患儿血清中Th17细胞相关细胞因子表达水平及意义 被引量:2

摘要 目的探讨新生儿感染性肺炎患儿血清辅助性17(Th17)细胞相关细胞因子白细胞介素17、22、23(IL-17、IL-22和IL-23)的表达水平及临床意义。方法选择我院新生儿科2015年1月?2016年12月收治的新生儿感染性肺炎住院患儿116例(重症组56例,轻症组60例)为研究对象,选择同期健康新生儿30例作为对照组。酶联免疫吸附(ELISA)法测定血清IL-17、IL-22和IL-23的表达水平,并与患儿肺炎严重程度及活动度进行相关性分析。结果患儿血清IL-17、IL-22和IL-23表达水平较对照组升高[(88.75±15.31)ng/L vs(67.98±7.55)ng/L、(151.94±14.51)ng/L vs(124.15±8.27)ng/L、(110.04±10.99)ng/L vs(86.54±6.71)ng/L];重症组显著高于轻症组[(94.11±16.47)ng/L vs(83.76±10.94)ng/L、(158.26±16.18)ng/L vs(146.04±9.65)ng/L、(114.35±11.80)ng/L vs(106.01±8.46)ng/L],并高于对照组(P<0.05)。恢复期IL-17、IL-22和IL-23的表达较急性期降低,但IL-22的表达差异无统计学意义[(73.73±1.72)ng/L vs(88.75±15.31)ng/L、(141.79±14.49)ng/L vs(151.94±14.51)ng/L、(96.46±10.59)ng/L vs(110.04±10.99)ng/L]。患儿血清IL-17、IL-22和IL-23之间呈显著正相关(r=0.677、0.632、0.740,P<0.01),并且其表达水平与肺炎严重程度及活动度正相关。结论 Th17细胞相关细胞因子(IL-17、IL-22和IL-23)在感染性肺炎患儿血清中存在差异表达,可做为患儿肺炎严重程度及疾病活动度的临床监测指标。 Objective To investigate the expression of interleukin-17,interleukin-22 and interleukin-23 in serum associated with Thl7 cells in neonates with pneumonia and its clinical significance.Methods A total of 116 hospitalized children with infectious pneumonia(56 cases in severe group,60 cases in mild group)were selected from Jan 2015 to Dec 2016 in our hospital,and 30 healthy neonates were selected as control group.The expression of IL-17,IL-22 and IL-23 in serum was determined by enzyme linked immunosorbent assay(ELISA),and the correlation between the severity and activity of pneumonia in children was analyzed.Results The expression levels of serum IL-17,IL-22 and IL-23 increased compared with the control group[(88.75±15.31)ng/L vs(67.98±7.55)ng/L,(151.94±14.51)ng/L vs(124.15±8.27)ng/L,(110.04±10.99)ng/L vs(86.54±6.7l)ng/L];those was significantly higher in severe group than in mild group and the control group[(94.11±16.47)ng/L vs(83.76±10.94)ng/L,(158.26±16.18)ng/L vs(146.04±9.65)ng/L,(114.35±11.80)ng/L vs(106.01±8.46)ng/L].The expression of IL-17,IL-22 and IL-23 during the recoveiy period decreased compared with acute period,but the level of IL-22 had no significant difference[(73.73±1.72)ng/L vs(88.75±15.31)ng/L,(141.79±14.49)ng/L vs(151.94±14.51)ng/L,(96.46±10.59)ng/L vs(110.04±10.99)ng/L].There was significant positive correlation among serum IL-17,IL-22 and IL-23^*=0.677,0.632,0.740,P<0.01),and the level of expression was positively correlated with the severity and activity of pneumonia.Conclusion Thl7 cell-associated cytokines(IL-17,IL-22 and IL-23)are differentially expressed in the serum of children with neonatal infectious pneumonia and can be used as clinical monitoring index for the severity and activity of pneumonia.
