老年患者股骨粗隆间骨折不同手术时机对预后影响的效果分析

Effect of different operation time on prognosis of intertrochanteric fracture in elderly patients

目的观察并分析不同手术时机治疗老年患者股骨粗隆间骨的应用成效。方法选自我院2013~2017年收治的66例老年股骨粗隆间骨折患者为研究对象,将患者随机分为两组,分别为对照组与观察组。对照组为发病48h内进行手术患者,观察组为发病48h后患者,两组患者均采用股骨近端防旋髓内钉固定手术,对两组患者治疗后的临床应用成效和预后成效进行观察研究。结果观察组患者的并发症发生率为6.06%,明显低于对照组的39.39%,差异有统计学意义(P<0.05);观察组患者术后一个月的死亡率为3.03%、一年的死亡率为15.15%,明显低于对照组患者的9.09%和24.24%,两组预后差异有统计学意义(P<0.05)。结论对老年股骨粗隆间骨折患者进行早期的股骨近端防旋髓内钉固定手术治疗方式,能有效改善患者的髋关节功能,提升其康复速度,充分体现其价值意义,值得临床领域不断采纳与推广。

Objective To observe and analyze the effects of different operative timing on the treatment of femoral intertrochanteric bone in elderly patients.Methods Sixty-six elderly patients with intertrochanteric fracture were selected from our hospital from 2013 to 2017.These patients were randomly divided into two groups,the control group and the observation group.The patients in the control group were operated within 48 hours,and the patients in the observation group were operated 48 hours after onset.Both groups were treated with proximal femoral antirotation intramedullary nail fixation.Results The incidence of complications in the observation group was 6.06%,significantly lower than that in the control group(39.39%),the difference was statistically significant(P<0.05).The mortality rate of the observation group was 3.03%one month after operation and 15.15%one year after operation,which was significantly lower than 9.09%and 24.24%in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Early proximal femoral antirotation intramedullary nail fixation for elderly patients with intertrochanteric fracture can effectively improve the hip function of patients,improve the recovery speed,fully reflect its value,and is worthy of continuous adoption and promotion in clinical field.
