超敏C-反应蛋白检测在儿童细菌和支原体感染中的应用研究

The application of hs CRP detection in diagnosis of bacterial and mycoplasma infection in children

目的探讨超敏C反应蛋白(hs-CRP)、白细胞计数和中性粒细胞比例三种指标对儿童细菌性感染和支原体感染的诊断价值。方法选择就诊的135例感染患者为病例组,其中细菌性感染97例(A组),支原体感染38例(B组),另外选择同期在医院进行健康体检的正常儿童60例作为对照组(C组),对hs-CRP、白细胞计数和中性粒细胞比例三种指标对细菌性感染和支原体感染的诊断价值进行评估。结果治疗前,3组患者的hs-CRP、白细胞计数和中性粒细胞比例和异常儿童数量的比例比较差异有统计学意义(P<0.05),在hs-CRP和白细胞计数中,A组显著高于另外2组,而在中性粒细胞比例中,B组显著高于另外2组,对A组患者进行7 d治疗后的hs-CRP、白细胞计数和异常儿童数量均显著下降,差异有统计学意义(P<0.05),利用hs-CRP对患者进行是否出现细菌感染的诊断价值最佳。结论 hs-CRP和白细胞计数在儿童细菌性感染的诊断和治疗效果观察中,均具有一定的指导意义;相对而言,hs-CRP的诊断价值更高,但是对于支原体感染的诊断,中性粒细胞比例则有一定的意义,对于儿童感染的诊断由于指标众多,需要综合考虑。

Objective To evaluate the application value of three indexes including high sensitivity C-reactive protein(hsCRP),white blood cell count and neutrophil ratio in diagnosis of bacterial infection and mycoplasma infection in children.Methods A total of 135 children patients with infection who were treated in our hospital from January 2011 to October 2016 were enrolled as case group,in whom,there were 97 cases of bacterial infection(group A)and 38 cases of mycoplasma infection(group B),at the same time,60 healthy children who underwent physical examination in our hosptital were enrolled as control group(group C).The diagnostic value of three indexes including hsCRP,white blood cell count and neutrophil proportion in diagnosis of bacterial and mycoplasma infection was evaluated.Result Before treatment,there were significant differences in hs CRP,white blood cell count and neutrophil proportion among the three groups(P<0.05).The levels of hs CRP and white blood cell count in group A were significantly higher than those in the other two groups,however,the levels of neutrophils proportion in group B were significantly higher than those in group A and group C.After 7-day treatment,the levels of hsCRP and white blood cell count were significantly decreased,as compared with those before treatment(P<0.05).The diagnostic value of hsCRP in diagnosis of bacterial infection was the best.Conclusion HsCRP and white blood cell count have a certain guiding significance in diagnosis and observation of therapeutic effects in children with bacterial infection,comparatively speaking,hs CRP has more important diagnostic value.However,neutrophil proportion has a certain significance in diagnosis of mycoplasma infection.
