鲍曼不动杆菌的分布及耐药性分析

DISTRIBUTION AND DRUG RESISTANCE OF ACINETOBACTER BAUMANNII

目的:通过了解重症监护病房(ICU)中鲍曼不动杆菌感染的分布特征以及鲍曼不动杆菌的耐药状况,为临床的防治以及抗菌药物的正确使用提供参考依据。方法:收集我院重症监护病房(ICU)1762例病人病例资料,选择病人的痰、中断尿、分泌物、中心静脉导管等部位标本的细菌培养结果,对培养分离到的鲍曼不动杆菌的分布和细菌耐药的状况进行了分析。结果:1762例病人中检出感染菌892株,其中鲍曼不动杆菌129株;自2014~2016鲍曼不动杆菌在ICU的检出率呈逐年上升趋势,由11.68%上升至16.31%;129株鲍曼不动杆菌主要分布在下呼吸道,约81.40%;药敏结果是对多种抗菌药物耐药,并且耐药率也是逐年上升,对丁胺卡那霉素耐药率最低。结论:ICU病房的鲍曼不动杆菌检出率呈现逐年上升,耐药情况比较严重,需要加强监测和积极干预,合理使用抗菌药物,医护人员要严格遵守无菌操作,减少鲍曼不动杆菌感染以及交叉感染发生。

Objective:By investigating the distribution characteristics and drug resistance of Acinetobacter baumannii in intensive care unit(ICU),it provides a reference for clinical prevention and treatment,furthermore,for proper use of antimicrobial agents.Methods:The clinical data of 1762 patients were collected in our hospital ICU.We collected the bacterial culture results of sputum,middle urine,secretions and central venous catheters and analyzed distribution and bacterial resistance.Results:Among the 1762 cases,892 cases of patients infected with bacteria were detected,where 129 cases are Acinetobacter baumannii;The 129 strains of Acinetobacter baumannii are mainly distributed in the lower respiratory tract,accounting for about 81.40%and the detection rate of Acinetobacter baumannii in ICU increased from 11.68%to 16.31%from 2014 to 2016.The drug susceptibility results were that they were resistant to a variety of antibiotics,and the drug resistance rate is also increased year by year.Besides,the resistance to amikacin is the lowest.Conclusion:The detection rate of Acinetobacter baumannii in ICU is increasing year by year,and the drug resistance is much more serious.It is necessary to strengthen the monitoring and active intervention and the rational use of antibiotics.The doctor and nurse should comply with the aseptic operation strictly and reduce Acinetobacter baumannii Infection and cross-infection.
