股骨粗隆间骨折两种治疗方法比较

The Comparison of Two Surgical Therapys in Femoral Intertrochanteric Fractures

目的:比较PFNA(股骨近端防旋髓内钉)和螺纹针-外固定架技术治疗股骨粗隆间骨折的效果。方法:回顾性分析笔者所在科2014年10月-2016年9月采用上述两种技术治疗的49例股骨粗隆间骨折患者的临床资料,其中PFNA治疗24例(1例患者先行螺纹针-外固定架手术治疗后出现螺纹针松动,改为PFNA治疗),螺纹针-外固定架治疗25例。统计分析并比较两组患者手术时间、手术出血量、术后引流量、骨折愈合时间、术后并发症、髋关节功能优良率等方面的差异。结果:A组患者手术时间、手术出血量、术后引流量、骨折愈合时间、术后并发症、髋关节功能优良率均优于B组,差异均有统计学意义(P<0.05)。结论:PFNA组在骨折愈合时间、术后并发症、骨折优良率等指标上优于螺纹针-外固定架组,同时,在手术时间、手术出血量、术后引流等指标上PFNA组也优于螺纹针-外固定架组。

Objective:To compare the effect in femoral intertrochanteric fractures between PFNA and screw-needles and external fixator.Method:The clinical data of 49 cases of femoral intertrochanteric fractures treated in two surgical therapys from October 2014 to September 2016 were analyzed retrospectively.It concludes that 24 cases were treated by PFNA(One case was treated firstly by screw-needles and external fixator,but lately was treated by PFNA for screw-needles loosening)and 25 cases treated by screw-needles and external fixator.Statistical analysis and comparion in two groups show the differences in operation time,operative blood loss,drainage after operation,postoperative complications,healing time,Excellent and good rate of hip.Result:The operation time,bleeding volume,postoperative drainage volume,fracture healing time,postoperative complications and hip joint function rate of the A group were significantly better than those in the B group,the differences were statistically significant(P<0.05).Conclusion:Group PFNA is better than screw thread external fixator in fracture healing time,postoperative complications and excellent and good rate of fracture.Meanwhile,PFNA group is better than screw thread external fixator group in terms of operative time,bleeding volume and postoperative drainage.
