经尿道等离子双极电切治疗良性前列腺增生的应用效果及手术方式探析

Transurethral Bipolar Plasma Cutting in the Treatment of Benign Prostatic Hyperplasia Application Effect and Operation Mode Analysis

目的探究经尿道等离子双极电切治疗良性前列腺增生的应用效果及手术方式。方法方便选取2016年2月—2017年3月福建医科大学附属宁德市医院泌尿外科90例良性前列腺增生患者,根据随机表分组。每一组45例。对照组采取传统的经尿道良性前列腺增生电切治疗;观察组采用经尿道等离子双极电切治疗。比较两组患者良性前列腺增生转归率;术中操作指标和出院时间;继发出血、膀胱颈挛缩、尿道狭窄等不良事件出现率;治疗前后排尿相关指标。结果观察组患者良性前列腺增生转归43例(95.56%),比对照组36例(80.00%)高(χ~2=5.075,P<0.05);观察组术中操作指标和出院时间比对照组好,观察组血量、膀胱冲洗时间、导尿管留置时间、出院时间分别(297.25±10.91)mL、(22.13±2.71)h、(96.05±4.57)h、(12.24±2.72)d优于对照组(145.24±5.85)mL、(12.31±1.53)h、(81.11±3.92)h、(9.11±1.53)d,差异有统计学意义(t=6.724、9.155、8.791、9.177,P<0.05)。观察组继发出血、膀胱颈挛缩、尿道狭窄等不良事件出现2例(4.44%)比对照组9例(20.00%)低(χ~2=5.075,P<0.05)。干预前排尿相关指标相似差异无统计学意义(P>0.05)。干预后观察组排尿相关指标优于对照组(P<0.05)。结论经尿道等离子双极电切治疗良性前列腺增生的应用效果确切,可有效减少出血,减少并发症,加速术后康复,改善排尿功能,值得推广应用。

Objective This paper tries to investigate the effect and surgical application of transurethral plasma bipolar electrotomy for benign prostatic hyperplasia(BPH).Methods 90 patients with benign prostatic hyperplasia(BPH)from February 2016 to March 2017 in Ningde Hospital Affiliated to Fujian Medical University were convenient collected and divided into groups according to random table,45 cases in each group.The control group adopted the traditional transurethral resection of benign prostatic hyperplasia;observation group adopted transurethral plasma bipolar electrotomy.The rates of benign prostatic hyperplasia(BPH)were compared between the two groups.The operation indexes and discharge time during operation were also compared.The occurrence rates of secondary bleeding,bladder neck contracture and urethral stricture were compared before and after treatment.Results In the observation group,the positive rate of benign prostatic hyperplasia in the observation group was 43 cases(95.56%)higher than that of the control group 36 cases(80.00%)(χ2=5.075,P<0.05)The bladder flushing time,catheter indwelling time and discharge time were(297.25±10.91)mL,(22.13±2.71)h,(96.05±4.57)h and(12.24±2.72)d respectively,(45.24±5.85)mL,(12.31±1.53)h,(81.11±3.92)h,(9.11±1.53)d,respectively(t=6.724,9.155,8.791,9.177,P<0.05).The incidence of secondary bleeding,bladder neck contracture and urethral stricture in observation group was lower than that in control group[2 cases(4.44%)vs 9 cases(20.00%)],the difference was statistically significant(χ2=5.075,P<0.05).Urine related indicators before intervention were similar the difference was no statistically significant(P>0.05).After intervention,urinary excretion-related indexes in observation group were better than those in control group(P<0.05).Conclusion Transurethral plasma bipolar resection of benign prostatic hyperplasia of the application of the exact effect,which can effectively reduce bleeding,reduce complications,accelerate postoperative rehabilitation,and improve urinary function.It is worth promoting the application.
