损伤控制性手术在严重肝脏外伤治疗中的应用分析

Application of Controlled Injury in the Treatment of Severe Liver Injury

目的对严重肝脏外伤患者应用损伤控制性手术的临床应用效果进行探究。方法收集自2011年3月—2017年7月间在该院接受肝脏外伤诊治的患者资料,从中随机选择34例患者作为研究对象,根据手术方式的不同将所选患者分为参照组和探究组,两组各包括17例患者,参照组患者接受常规治疗,探究组患者接受损伤控制性手术治疗方式,对比2组患者手术治疗情况和术后并发症发生情况。结果 2组患者手术时间差异无统计学意义(t=0.292 8,P=0.770 3),探究组患者术后恢复时间明显短于参照组患者且病死率更低,2组患者上述观察指标均差异有统计学意义(P<0.05)。探究组4例患者出现并发症,所占比重为23.53%,参照组8例患者出现并发症,所占比重为47.06%,对比2组患者术后并发症发生率差异有统计学意义(χ2=4.371 4,P=0.036 5)。结论对严重肝脏外伤患者应用损伤控制性手术能够取得理想的临床疗效,同时可有效控制并发症发生率。

Objective This paper tries to investigate the clinical application of trauma-controlled surgery in patients with severe liver trauma.Methods From March 2011 to July 2017 in this hospital for treatment of liver injury in patients with information from 34 patients were randomly selected as the study object,according to the different surgical methods,they were divided into the selected group of patients and the control group,with 17 patients in each group.The patients in the control group received routine treatment,and the patients in the study group received controlled surgery.The surgical treatment and postoperative complications were compared between the two groups.Results There was no significant difference between the two groups in the operation time(t=0.292 8,P=0.770 3).The recovery time of the study group was significantly shorter than that of the reference group and the mortality rate was lower.The above-mentioned indexes There was a significant difference(P<0.05).In the study group,4 patients had complications,accounting for 23.53%.Complications were found in 8 patients in the reference group,accounting for 47.06%.The incidence of postoperative complications was significantly different between the two groups(χ2=4.371 4,P=0.036 5).Conclusion The application of injury control surgery in patients with severe liver trauma can achieve the desired clinical efficacy,and can effectively control the incidence of complications.
