2型糖尿病患者微量元素铁水平与糖尿病肾病发生风险的临床关系分析 被引量:5

摘要 目的:对2型糖尿病患者微量元素铁水平与糖尿病肾病发生风险的临床关系进行探究。方法:收治2型糖尿病患者80例,根据病情分A组(2型糖尿病无肾病患者)、A1组(糖尿病肾病Ⅲ期)、A2组(糖尿病肾病Ⅳ期)。对不同组微量元素铁水平进行检测。结果:从A组到A1组到A2组,收缩压(SBP)、舒张压(DBP)、尿素氮(BUN)、尿酸(UA)、血清铁逐渐上升,3组SBP与血清铁差异有统计学意义(P<0.05);从A组到A1组到A2组,肾小球滤过率(GFR)逐渐下降,差异有统计学意义(P<0.05);GFR和年龄、BUN、UA、血清铁呈负相关(P<0.05)。结论:血清微量元素铁水平、年龄、BUN、UA是影响2型糖尿病患者肾病发生风险的因素,需要在早期对这些因素进行监控。 Objective:To study the clinical relationship between the level of iron and the risk of diabetic nephropathy in patients with type 2 diabetes.Methods:80 patients with type 2 diabetes were selected.According to the condition of the disease,the A group(type 2 diabetes without nephrotic patients),the group A1(diabetic nephropathy stageⅢ),the A2 group(stageⅣdiabetic nephropathy).The iron level of different groups of trace elements was detected.Results:From the group A to the group A1 and to the group A2,the systolic pressure(SBP),diastolic pressure(DBP),urea nitrogen(BUN),uric acid(UA),and serum iron increased gradually.The difference in the 3 groups of SBP and serum iron was statistically significant(P<0.05).From the group A to the group A1 and to the group A2,the glomerular filtration rate(GFR)decreased gradually,and the difference was statistically significant(P<0.05).GFR was negatively correlated with age,BUN,UA and serum iron(P<0.05).Conclusion:Trace element serum iron level,age,BUN and UA are the risk factors of nephropathy in patients with type 2 diabetes mellitus.These factors need to be monitored in early stage.
