女性不同类型尿失禁的电生理差异分析 被引量:2

摘要 目的探讨女性不同类型尿失禁(Urinary Incontinence,UI)的电生理差异。方法收集2014年8月至2017年10月就诊于大连市妇女儿童医疗中心的女性尿失禁患者331例,其中诊断压力性尿失禁(Stress Urinary Incontinence,SUI)210例,急迫性尿失禁(Urge Urinary Incontinence,UUI)50例,混合性尿失禁(Mixed Urinary Incontinence,MUI)71例。各组选取50例为试验组,选取同期就诊的健康体检女性50例为对照组;采用法国Phenix USB 2检测研究对象的盆底电生理参数:盆底肌的肌力、最大肌电位值、疲劳度。结果 SUI、UUI、MUI组的I类及II类肌力均小于正常对照组(P<0.05);SUI、UUI、MUI组I及II类肌疲劳度均大于对照组(P<0.05);SUI、UUI、MUI组的最大肌电位值分别为:(11.46±6.85)μV,(16.31±2.36)μV,(14.67±4.02)μV。对照组最大肌电位值为(17.62±2.31)μV。SUI组及MUI组肌电位值明显小于对照组(P<0.05)。UUI肌电位值大于SUI、MUI组(P<0.05)。结论女性不同类型尿失禁的电生理参数存在差异,盆底电生理指标检测能为女性尿失禁的诊断及治疗提供一定的参考价值。 Objective To analyze and discuss the electrophysiological differences of different types of urinary incontinence in female patients.Methods Totally,331 patients with urinary incontinence were enrolled in the study from 08/2014 to 10/2017 in Dalian municipal women and children's medical center,including 210 patients with Stress Urinary Incontinence(SUI),50 with Urge Urinary Incontinence(UUI),and 71 with Mixed Urinary Incontinence(MUI).Fifty patients from each type of urinary incontinence patients were selected as the experimental group;fifty healthy women were selected in our center as the control group.Pelvic floor electrical physiological parameters were detected by the French Phenix USB 2.Pelvic floor electrical physiological parameters contained Muscle strength,maximum muscle potential,and fatigue.Results The muscle strength of type I and type II in the SUI,UUI,and MUI groups was lower than that in the control group(P<0.05),the muscle fatigue of type I and type II in the SUI,UUI,and MUI groups was higher than that in the control group(P<0.05).The maximum muscle electromyographic values in the control,SUI,UUI and MUI groups were(17.62±2.31)μV,(11.46±6.85)μV,(16.31±2.36)μV and(14.67±4.02)μV.The electromyographic values of SUI group and MUI group were significantly lower than those of the control group(P<0.05).Electromyographic value of UUI group was greater than of SUI and MUI groups(P<0.05).Conclusion Electrophysiological parameters of different types of urinary incontinence are different,and the detection of pelvic floor electrophysiological indexes can provide certain reference value for the diagnosis and treatment of urinary incontinence.
