裸支架植入治疗孤立性肠系膜上动脉夹层临床研究

Treatment of isolated superior mesenteric artery dissection with bare-metal stent implantation

目的探讨裸支架植入治疗孤立性肠系膜上动脉夹层的应用价值。方法收集2014年8月至2017年2月长沙市第四医院7例自发孤立性肠系膜上动脉夹层行裸支架植入患者资料,通过患者临床症状及CTA检查,评价治疗效果。结果 7例病人术后均有临床症状改善,随访2~25个月(平均18.7月)均未出现夹层破裂死亡、血管闭塞致肠坏死等严重并发症,部分存在轻度肠缺血症状。结论裸支架植入可作为部分自发孤立性肠系膜上动脉夹层治疗方案的良好选择。

Objective To evaluate the value of bare-metal stent placement in the treatment of isolated superior mesenteric artery dissection.Methods Clinical data were collected on the patients with spontaneous isolated superior mesenteric artery dissection who were treated with bare–metal stent placement from August 2014 to February 2017 in the Fourth Hospital of Changsha.The patient's symptoms,signs and CTA were observed,and outcome was evaluated.Results The clinical symptoms were improved after operation in 7 patients.All patients were followed up for 2 to 25 months(mean 18.7 months).There were no severe complications such as rupture of the dissection,intestinal necrosis due to vascular occlusion.Mild intestinal ischemia was observed in some cases.Conclusion Bare-metal stent placement may be a good choice for selected spontaneous isolated mesenteric artery dissection.
