阻塞性睡眠呼吸暂停低通气综合征对急性脑梗死患者认知功能及心理健康状况的影响

Study on the relationship between strock and obstructive sleep apnea-hypopnea syndrome and mental health status in acute cerebral infarction patients

目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)对急性脑梗死(ACI)患者认知功能及心理健康状况的影响。方法 ACI患者83例,根据多导睡眠仪监测(PSG)结果分为OSAHS组37例,非OSAHS组46例,采用焦虑自评量表(SAS)、抑郁自评量表(SDS)和蒙特利尔认知评估量表(MoCA)进行测评。结果 ACI合并OSAHS组SAS焦虑状态总分、SDS的精神情感症状、躯体化障碍、抑郁的心理障碍和总分与对照组比较明显增高,差异有统计学意义(P<0.05)。ACI合并OSAHS组蒙特利尔认知评估量表(MoCA)中视空间和执行能力、注意力、语言能力、抽象能力、延迟记忆能力、命名能力和总分与对照组比较明显降低,差异有统计学意义(P<0.05)。结论 ACI合并OSAHS患者更易出现不同程度的焦虑抑郁症状;ACI合并OSAHS会加重患者的认知功能障碍。

Objective To investigate the influence of obstructive sleep apnea-hypopnea syndrome(OSAHS)on cognition and mental health status for patients with acute cerebral infarction(ACI).Method Eight-seven patients with ACI were recruited in our study.According to the results of polysomnography(PSG),those participants were divided into two groups,one of which was patients with OSAHS(number=37 patients)and the other was non-OSAHS(NOSHS,number=46 patients).Both groups carried on the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)and Montreal Cognitive Assessment(MoCA).Results The statistically significantly differences were found on the the SAS and SDS between the OSAS group and the NOSHS group(P<0.01).Compared with NOSAS group,patients with OSAS had significantly decline on the cognitive scores,including visual space and executive capacity,attention,language,abstract capacity,delayed memory,naming,and total scores(P<0.05).Conclusion The anxiety,depression were found to be high prevalent among ischemic stroke patients with OSAHS.OSAHS can cause cognitive impairment in patients with ACI.
