痰热清注射液联合无创正压通气治疗COPD急性期的应用效果研究

Effects of Tanreqing Injection Combined with Noninvasive Positive Pressure Ventilation on Acute Exacerbation of COPD

目的:探究痰热清注射液联合无创正压通气(NIPPV)治疗在慢性阻塞性肺病急性期(AECOPD)中的应用效果。方法:将收治的82例AECOPD患者采用随机数表法分为研究组和对照组,每组41例。2组均予以NIPPV治疗,研究组加用痰热清注射液。观察2组患者疗效,评估患者治疗前及治疗1w后疾病相关指标[血清转化生长因子-β(TGF-β)、肿瘤坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)、基质金属蛋白酶-9(MMP-9)],治疗1w后患者健康状况[慢性阻塞性肺病测试(CAT)]。结果:(1)治疗1w后,研究组较对照组总显效率高(P<0.05),CAT得分低(P<0.05);(2)治疗1w后,2组TNF-α、hs-CRP、MMP-9水平均较治疗前低(P<0.05),TGF-β水平均较治疗前高(P<0.05),且研究组变化幅度大于对照组(P<0.05)。结论:痰热清联合NIPPV可有效控制COPD患者急性症状、提高患者健康状况,治疗AECOPD疗效佳。

Objective:To explore the effects of Tanreqing injection combined with noninvasive positive pressure ventilation(NIPPV)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:82 cases of AECOPD patients were randomly divided into the study group and the control group with 41 cases in each group according to the random number table method.The two groups were treated with NIPPV,and the study group was additionally given Tanreqing injection.The efficacy was observed in two groups,and the disease-related indexes[serum transforming growth factorβ(TGF-β),tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP),matrix metalloproteinase-9(MMP-9)]and health status of patients after 1w of treatment[chronic obstructive pulmonary disease assessment test(CAT)]were assessed before treatment and after 1w of treatment.Results:After 1w of treatment,the total effective rate in the study group was significantly higher(P<0.05)while CAT score was lower(P<0.05)compared with the control group.After 1w of treatment,the levels of TNF-α,hs-CRP and MMP-9 in the two groups were lower than those before treatment(P<0.05)while the level of TGF-βwas higher than that before treatment(P<0.05),and the change degree in the study group was higher than that in the control group(P<0.05).Conclusions:Tanreqing combined with NIPPV can effectively control the acute symptoms of COPD patients and improve health status of patients,and it has good efficacy in the treatment of AECOPD.
