甲状腺全切除术联合同位素治疗乳头状甲状腺癌对患者MMP-2、MMP-9和PTEN的影响

Effects of total thyroidectomy combined with isotope therapy on MMP-2,MMP-9 and PTEN in patients with papillary thyroid carcinoma

目的探讨应用甲状腺全切除术联合同位素治疗乳头状甲状腺癌对患者基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)和第10号染色体同源丢失性磷酸酶-张力蛋白基因(PTEN)的影响。方法选择2014年4月至2015年2月收治的45例乳头状甲状腺癌患者的临床资料,观察患者治疗后的疗效、并发症发生情况及术后随访情况,同时对比分析患者治疗前后MMP-2、MMP-9表达水平及血清P53、PTEN及缺氧诱导因子(HIF)-1α水平。结果45例患者均顺利完成手术,术中及住院期间无患者死亡,其治疗总有效率为95.55%;6例患者出现并发症,其发生率为13.33%;2例患者肿瘤复发,其发生率为4.44%。患者治疗后MMP-2、MMP-9表达水平较治疗前明显降低,差异有统计学意义(P<0.05)。患者治疗后血清P53、PTEN及HIF-1α水平较治疗前明显改善,患者治疗后血清P53及HIF-1α水平明显低于治疗前,而PTEN水平明显高于治疗前,差异均有统计学意义(P<0.05)。结论乳头状甲状腺癌患者应用甲状腺全切除术联合同位素进行治疗,疗效显著,对患者的MMP-2与MMP-9表达水平及血清P53、PTEN及HIF-1α水平均有明显的改善作用,且安全性较高。

Objective To investigate the Effects of total thyroidectomy combined with isotope therapy on matrix metalloproteinase-2(MMP-2),matrix metalloproteinase-9(MMP-9)and homologous loss of phosphatase on chromosome 10 in patients with papillary thyroid carcinoma-phosphatase and tensin homolog deleted from chromosome ten(PTEN)in patients with papillary thyroid carcinoma.Methods The clinical data of 45 patients with papillary thyroid carcinoma admitted from April 2014 to February 2015 were selected.The expression of MMP-2 and MMP-9 and the levels of serum P53,PTEN and HIF-1αwere measured before and after treatment.The levels of MMP-2 and MMP-9 were measured before and after treatment.Results A total of 45 patients were successfully completed surgery,intraoperative and hospital stay without patients died,the total effective rate of 95.55%;6 patients with complications,the incidence was 13.33%;2 patients with tumor recurrence,the incidence was 4.44%.The expression of MMP-2 and MMP-9 in patients treated with treatment was significantly lower than that before treatment(P<0.05).The levels of P53,PTEN and HIF-1αin the patients were significantly improved compared with those before treatment.The levels of serum P53 and HIF-1αwere significantly lower than those before treatment,while the level of PTEN was significantly higher than that before treatment statistically significant(P<0.05).Conclusion For papillary thyroid cancer patients,thyroidectomy combined with isotope has betten effect,which could improve the expression of MMP-2 and MMP-9 in serum,as well as the levels of serum P53,PTEN and HIF-1αand it has higher security.
