摘要
目的研究炎症及炎症相关因子对老年人衰弱的影响。方法年龄≥70岁的老年慢病稳定期患者作为研究对象,运用老年衰弱FRAIL评分方法将其分为对照组和衰弱组,每组34例。2组患者分别于入院24 h内进行血液肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C-反应蛋白(hs-CRP)、纤维蛋白原(FIB)和血沉(ESR)检测。结果与对照组比较,衰弱组患者血液中TNF-α、IL-6和hs-CRP水平显著增加(P<0.05)。Pearson相关分析显示,老年衰弱患者血液中IL-6和hs CRP含量与衰弱评分呈正相关(R^2=0.357、0.131,P<0.05)。与对照组比较衰弱组患者血液中炎症相关的FIB显著增加、ESR显著延长(P<0.05)。结论老年衰弱患者体内存在低水平的慢性炎症反应,炎症或炎症相关因子的监控将为衰弱的诊断以及疾病预后判断提供有价值的参考。
Objective To investigate the effects of inflammation and inflammatory-associated factors on Marasmus in the elderly.Methods The elderly patients with senile chronic diseases aged more than 70 years who were admitted to our hospital from October 2016 to December 2017 were enrolled as research subjects in the study,who were divided into control group(n=34)and Marasmus group(n=34)according to FRAIL score.The levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),fibrinogen(FIB)and blood sedimentation(ESR)within 24 hours after admission to hospital were detected and compared between the two groups.Results As compared with those in control group,the levels of TNF-α,IL-6 and hs-CRP in Marasmus group were significantly increased(P<0.05).Pearson correlation analysis showed that the levels of IL-6 and hs-CRP in Marasmus group were positively correlated with FRAIL scores(R 2=0.357,0.131,respectively,P<0.05).As compared with those in control group,the levels of FIB in Marasmus group were significantly increased,and ESR time was significantly prolonged(P<0.05).Conclusion The low level chronic inflammatory response exiats in the elderly frail patients,therefor,the monitoring of inflammation and inflammatory-associated factors will provide a valuable reference for the diagnosis of Marasmus and prognosis of patients.
