富自体生长因子纤维蛋白凝胶对上颌骨囊肿切除术后骨缺损修复的长期疗效评估

The long-term therapeutic effects of concentrated growth factors fibrin gelatin on bone defect repair after maxillary cyst resection

目的探讨自体牙骨粉、羟基磷灰石以及富自体生长因子纤维蛋白凝胶对上颌骨囊肿切除术后骨缺损修复的长期疗效。方法采用随机数字法将120例拟行上颌骨囊肿切除术患者随机分为A、B、C组,每组包含40例受试者,A、B、C组受试者上颌骨囊肿术中骨腔内分别植入自体牙骨、羟基磷灰石、富自体生长因子纤维蛋白凝胶。分别于术后3、6个月比较3组受试者骨缺损修复效果。结果 C组受试者术后3个月、6个月垂直骨吸收量、舌侧骨吸收量、平均灰度值、牙周袋深度以及附着丧失均明显优于A、B组,且A组优于B组,差异均有统计学意义(P<0.05)。结论三种材料均具有较好的骨缺损修复效果,可有效降低创骨吸收,促进创面愈合,其中富自体生长因子纤维蛋白凝胶最优,自体牙骨粉优于羟基磷灰石。

Objective To investigate the long-term therapeutic effects of autogenous dental bone dust,hydroxyapatite and concentrated growth factors on bone defect repair after maxillary cyst resection.Methods A total of 120 patients with maxillary bone cyst were randomly divided into three groups,group A,B,C,with 40 patients in each group.The patients in group A,B,C were implanted with autologous dental bone,hydroxyapatite,concentrated growth factors fibrin gelatin,respectively.The repair effects of bone defect were observed and compared at 3m,6m after surgery.Results The vertical bone absorption,lingual bone absorption,average grey value,periodontal pocket depth,attachment loss at 3m,6m after surgery in group C were superior to those in group A and group B,which in group A were were superior to those in group B(P<0.05).Conclusion The three materials used in this study have good repair effects on bone defect,which can effectively reduce the bone absorption and promote wound healing,in which,the concentrated growth factors fibrin gelatin is the best,moreover,autogenous dental bone dust is superior to hydroxyapatite.
