无症状性心肌缺血的动态心电图检查及结果分析

Dynamic Electrocardiogram Examination and Results Analysis of Silent Myocardial Ischemia

目的对无症状性心肌缺血的动态心电图检查结果进行分析。方法选取我院2017年1—9月收治的无症状性心肌缺血患者85例作为研究对象,并以同期收治的有症状性心肌缺血患者80例作为对照,分别对其实施常规心电图和动态心电图检测,比较两种方法诊断心肌缺血的阳性率,并对有症状性心肌缺血和无症状性心肌缺血患者的动态心电图检测差异进行对比分析。结果常规心电图和动态心电图诊断心肌缺血的阳性率分别为74.55%(144/165)和97.58%(161/165),动态心电图的诊断阳性率高于常规心电图,比较差异有统计学意义(P<0.05)。同时,动态心电图检测心肌缺血患者的结果显示,无症状性心肌缺血组患者的缺血持续时间显著低于有症状性心肌缺血,而心率则显著高于有症状性心肌缺血,比较差异有统计学意义(P<0.05)。结论动态心电图可以对心肌缺血患者进行持续监测,诊断无症状性心肌缺血的阳性率更高,有利于病情评估和治疗。

Objective To analyze the results of dynamic electrocardiogram(ECG)examination in silent myocardial ischemia.Methods 85 cases of asymptomatic myocardial ischemia admitted in our hospital from January to September 2017 were selected as the research subjects.80 patients with symptomatic myocardial ischemia received the same period as control.Routine electrocardiogram and dynamic electrocardiogram were tested respectively.The two methods were used to diagnose the positive rate of myocardial ischemia and to compare the differences of the dynamic electrocardiogram detection between symptomatic and asymptomatic myocardial ischemia.Results The positive rates of routine electrocardiogram(ECG)and dynamic electrocardiogram(ECG)in diagnosing myocardial ischemia were 74.55%(144/165)and 97.58%(161/165)respectively.The positive rate of dynamic electrocardiogram(ECG)was higher than that of routine ECG,and the difference was statistically significant(P<0.05).At the same time,the results of myocardial ischemia in patients with dynamic electrocardiogram showed that the duration of ischemic duration in the patients with asymptomatic myocardial ischemia was significantly lower than that of symptomatic myocardial ischemia.The heart rate was significantly higher than that of symptomatic myocardial ischemia,and the difference was statistically significant(P<0.05).Conclusion The dynamic electrocardiogram can be used to monitor the patients with myocardial ischemia continuously,and the positive rate of asymptomatic myocardial ischemia is higher,which is beneficial to the evaluation and treatment of the disease.
