“双荷包”吻合法预防食管癌术后吻合口狭窄疗效分析

Double Ring Suture Technique in Prevention of Postoperative Anastomotic Stricture for Esophageal Cancer

目的探讨改进胃食管吻合方法对预防食管癌术后吻合口狭窄的价值。方法选取2014-01—2015-12间在河南省胸科医院接受食管癌根治术(颈部吻合)的183例患者。将应用胃食管颈部器械"双荷包"吻合法的89例作为观察组,将同期应用常规器械吻合法的94例患者作为对照组。回顾性分析患者的临床资料。结果观察组吻合口狭窄的发生率低于常规器械吻合组,差异有统计学意义(P<0.05)。2组吻合口瘘及反流性食管炎发生率差异无统计学意义(P>0.05)。结论胃食管颈部器械"双荷包吻合法"可有效预防食管癌术后吻合口狭窄的发生。

Objective To elucidate a modified anastomosis method in the prevention of postoperative anastomotic stricture for esophagectomy for esophageal cancer.Methods Totally 183 patients from 2014-01 to 2015-12 underwent radical esophagectomy(cervical anastomosis)in Henan provincial chest hospital.89 cases of gastroesophageal neck instruments with double bag anastomosis method were selected as the observation group.94 patients who received routine instrument anastomosis were used as control group.Retrospective analysis of the clinical data of the patients.Results The incidence of anastomotic stenosis in the observation group was lower than that in the conventional instrument anastomosis group(P<0.05).There was no significant difference in the incidence of anastomotic leakage and reflux esophagitis between the 2 groups(P>0.05).Conclusion Gastroesophageal neck device double bag anastomosis method can effectively prevent the occurrence of anastomotic stenosis after esophagus cancer operation.
